摘 要:目的 探索极低/超低体质量新生儿出生后促甲状腺素浓度水平及变化趋势。方法 回顾性分析2017—2018年新疆地区81520名新生儿筛查数据,剔除不完整数据与CH确诊患儿数据,其余新生儿按照出生体质量分为极低/超低体质量组、低体质量组、正常体质量组、超重组,再将各组按采血日龄分为3~7天采血组与超7天采血组,分析各组TSH浓度差异。结果 符合纳入标准的新生儿共计81405人。分析结果显示极低/超低体质量组中3~7天采血组TSH浓度低于超7天采血组,而正常体质量组与超重组中3~7天采血组TSH浓度高于超7天采血组,差异均具有极显著性(P<0.01);3~7天采血组中极低/超低体质量组TSH浓度低于低体质量组、正常体质量组与超重组,超7天采血组中极低/超低体质量组与低体质量组TSH浓度高于正常体质量组与超重组,差异均具有显著性(P<0.05)。结论 极低/超低体质量新生儿出生早期TSH浓度低于正常体质量/超重新生儿,但在出生7天后可能存在延迟升高,其切值的设定应扩大数据量进一步分析研究。Objective To explore the thyroid stimulating hormone levels and trends of very low/extremely low birth weight newborns. Methods The screening data of 81520 newborns in Xinjiang from 2017 to 2018 were retrospectively analyzed. After excluding incomplete data and data of children diagnosed, the rest newborns were divided into very low/extremely low birth weight group, low birth weight group, normal birth weight group and overweight group according to birth weight.Each group was divided into 3-7 days blood sampling group and over 7 days blood sampling group according to the age of blood sampling. The difference of TSH concentration in each group was analyzed. Results A total of 81405 newborns met the inclusion criteria. The results showed that the TSH concentrations in very low/extremely low birth weight newborns with 3-7days blood sampling group was lower than that with over 7 days blood sampling group, while the TSH concentration in normal birth weight and overweight newborns with 3-7 days blood sampling group was higher than that with over 7 days blood sampling group, with significant difference(P<0.01). In addition, the TSH concentration of very low/extremely low birth weight newborns in 3-7 days blood collection group was lower than that of low birth weight, normal birth weight and overweight newborns, and that of very low/extremely low birth weight and low birth weight newborns in over 7 days blood collection group was higher than that of normal birth weight and overweight newborns, with significant differences(P<0.05). Conclusion The TSH concentration of very low/extremely low birth weight infants is lower than that of normal/overweight infants, but there may be delayed increase after 7 days of birth. The setting of its cut-off value should be further analyzed and studied by expanding the amount of data.
