益肾化湿颗粒联合骨化三醇对糖尿病肾病患者的临床疗效

摘　　要：目的考察益肾化湿颗粒联合骨化三醇对糖尿病肾病患者的临床疗效。方法120例患者随机分为对照组和观察组,每组60例,对照组给予骨化三醇,观察组在对照组基础上加用益肾化湿颗粒,疗程2个月。检测临床疗效、肾功能指标(BUN、Cr、β2-MG、Cys-C、尿白蛋白排泄率)、血脂指标(TC、HDL-C、LDL-C)、氧化应激指标(SOD、MDA、TNF-α、IL-6)、胰岛β细胞功能指数(FPG、FINS、HOMA-β)、不良反应发生率变化。结果观察组总有效率高于对照组(P<0.05)。治疗后,2组HDL-C、SOD、HOMA-β升高(P<0.05),肾功能指标、TC、LDL-C、MDA、TNF-α、IL-6、FPG、FINS降低(P<0.05),以观察组更明显(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论益肾化湿颗粒联合骨化三醇可安全有效地缓解糖尿病肾病患者临床症状,其机制可能与改善氧化应激水平、提高胰岛素β细胞功能指数有关。AIM To investigate the clinical effects of Yishen Huashi Granules combined with calcitriol on patients with diabetic nephropathy.METHODS One hundred and twenty patients were randomly assigned into control group(60 cases)for 2-month administration of calcitriol,and observation group(60 cases)for 2-month administration of both Yishen Huashi Granules and calcitriol.The changes in clinical efficacy,renal function indices(BUN,Cr,β2-MG,Cys-C,excretion rate of urinary albumin),blood lipid indices(TC,HDL-C,LDL-C),oxidative stress indices(SOD,MDA,TNF-α,IL-6),insulinβ-cell function indices(FPG,FINS,HOMA-β)and incidence of adverse reactions were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed increased HDL-C,SOD and HOMA-β(P<0.05),and increased renal function indices,TC,LDL-C,MDA,TNF-α,IL-6,FPG and FINS(P<0.05),especially for the observation group(P<0.05).No significant difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with diabetic nephropathy,Yishen Huashi Granules combined with calcitriol can safely and effectively alleviate clinical symptoms,whose mechanisms may contribute to the improvement of oxidative stress levels and enhancement of insulinβ-cell function indices.
