摘 要:目的对妊娠糖尿病(gestational diabetes mellitus,GDM)孕妇肠道菌群的丰度以及多样性进行评估分析,判断其与发病之间的关联,判断肠道菌群与病因之间的关联。方法随机选取19例孕24~28周的GDM患者作为实验组,19例健康孕妇作为对照组。对所有受试者粪便标本中的肠道分离获得的肠道细菌16S rRNA基因的V4区分别进行高通量的测序和生物信息学分析。结果两组研究对象肠道菌群的丰富度以及多样性大致相同。实验组厚壁菌门以及放线菌门的含量均明显少于对照组(χ^(2)=729.902,P<0.001;χ^(2)=26.880,P<0.001),而拟杆菌门以及变形菌门的含量高于对照组(χ^(2)=428.233,P<0.001;χ^(2)=545.707,P<0.001),实验组拟杆菌属、瘤胃球菌属、双歧杆菌属及乳酸杆菌属的含量均少于对照组(χ^(2)=171.939,P<0.001;χ^(2)=227.997,P<0.001;χ^(2)=26.926,P<0.001;χ^(2)=140.868,P<0.001),另外,实验组普氏菌属、埃希氏菌属、梭状芽胞杆菌属含量高于对照组(χ^(2)=250741.608,P<0.001;χ^(2)=576.128,P<0.001;χ^(2)=6.945,P=0.080)。结论GDM孕妇与健康孕妇之间,肠道菌群的丰度以及多样性大致相同,但是肠道菌群特异性又较大区别,GDM孕妇粪便中的放线菌门以及厚壁菌门比较少,而变形菌门和拟杆菌门量较多。Objective The abundance and diversity of intestinal flora in pregnant women with GDM were evaluated and analyzed to judge the relationship between it and the pathogenesis of GDM,and to judge the relationship between intestinal flora and the etiology of GDM.Methods In this study,38 pregnant women admitted to the obstetric clinic and endocrine Department of Heilongjiang Provincial Hospital were taken as the research objects.The gestational weeks of patients ranged from 24 weeks to 28 weeks,including 19 patients with GDM as the experimental group and 19 healthy pregnant women as the control group.Collect and sort out the clinical data of the research object,collect fecal samples,extract the flora DNA of fecal samples,carry out PCR amplification of V4 region of 16S rRNA gene,conduct high-throughput sequencing,and analyze bioinformatics at the same time.Results The richness and diversity of intestinal microflora in the two groups are roughly the same.The contents of Firmicutes and Actinomycetes in the experimental group were significantly lower than those in the control group(χ^(2)=729.902,P<0.001;χ^(2)=26.880,P<0.001),while the contents of Bacteroidea and Proteus were higher than those in the control group(χ^(2)=428.233,P<0.001;χ^(2)=545.707,P<0.001),Bacteroidea,rumen coccus the contents of Bifidobacterium and Lactobacillus in the experimental group were lower than those in the control group(χ^(2)=171.939,P<0.001;χ^(2)=227.997,P<0.001;χ^(2)=26.926,P<0.001;χ^(2)=140.868,P<0.001).In addition,the contents of Prevotella,Ehrlichia and Clostridium in the experimental group were higher than those in the control group(χ^(2)=250741.608,P<0.001;χ^(2)=576.128,P<0.001;χ^(2)=6.945,P=0.080).Conclusion Between GDM pregnant women and healthy pregnant women,the abundance and diversity of intestinal flora are roughly the same,but the specificity of intestinal flora is quite different.Actinomycetes and Firmicutes in the feces of GDM pregnant women are less,while Proteus and Bacteroides are more.
