摘 要:目的:探讨针刺“百会”透“曲鬓”治疗大鼠脑出血后脑水肿程度与炎症反应的影响以及miR-34a-5p在针刺治疗脑出血中的作用。方法:采用自体血注入尾壳核的方法制备脑出血大鼠模型,将60只造模成功的大鼠随机分为假手术组(Sham)、模型组(ICH)、模型+针刺治疗组(ICH+acupuncture)、模型+针刺治疗+agomir阴性对照组(ICH+acupuncture+agomir-NC)、模型+针刺治疗+miR-34a-5p agomir组(ICH+acupuncture+miR-34a-5p agomir),每组12只,每组随机分D1、D7两个时间点。建模12 h后针刺治疗,于相应时间点分别处死大鼠收集脑组织及血液,测算大鼠脑出血后脑组织的含水量、检测脑组织及血清中IFN-γ、IL-4的浓度水平变化。结果:脑含水量结果显示,与ICH组相比,ICH+acupuncture组各时间点脑干湿比均显著降低(P<0.01);与ICH+acupuncture组相比,ICH+acupuncture+miR-34a-5p agomir组脑干湿比显著升高(P<0.01);与ICH+acupuncture+agomir-NC组相比,ICH+acupuncture+miR-34a-5p agomir组脑干湿比显著升高(P<0.01)。ELISA结果显示,ICH组与ICH+acupuncture组比较,ICH+acupuncture组D7时脑组织及血清中IFN-γ浓度水平显著降低(P<0.01),IL-4浓度水平显著升高(P<0.01);与ICH+acupuncture组和ICH+acupuncture+agomir-NC组相比,ICH+acupuncture+miR-34a-5p agomir组D7时脑组织及血清中IFN-γ浓度水平显著升高(P<0.01),IL-4浓度水平显著降低(P<0.01)。结论:针刺“百会”透“曲鬓”能改善脑水肿程度,抑制促炎因子IFN-γ的释放,促进抗炎因子IL-4的释放。针刺“百会”透“曲鬓”可能通过抑制炎症反应过程,从而减轻脑水肿程度。过表达miR-34a-5p能减弱针刺“百会”透“曲鬓”对于脑出血后炎症反应及脑水肿的干预效果。Objective:To explore the effect of needling Baihui(DU20)-through-Qubin(GB7)on degree of brain edema and inflammatory response after cerebral hemorrhage(ICH)in rats,and to discuss the effect of miR-34a-5p on therapeutic effect of acupuncture in treatment of ICH.Methods:ICH rat model was established by injecting autologous blood into caudate putamen.60 rats with successful modeling were randomly divided into the sham operation group,the model group,the acupuncture group,the acupuncture plus agomir-NC group,and the acupuncture plus miR-34a-5p agomir group,with 12 rats in each group.Each group was randomly divided into D1 and D7 time points.Acupuncture intervention started 12 hours after modeling.Brain tissue and blood sample were collected at the corresponding time point to calculate the water content of brain tissue and to detect the serum concentrations of IFN-γand IL-4 in ICH rats.Results:Brain water content results showed that the brain stem wet ratio was significantly decreased in the acupuncture group compared with that in the model group(P<0.01);it was significantly increased in the acupuncture plus miR-34a-5p agomir group compared with that in the acupuncture group(P<0.01);it was significantly increased in the acupuncture plus miR-34a-5p agomir group compared with that in the acupuncture plus agomir-NC group(P<0.01).The results of ELISA showed that the concentrations of IFN-γin brain tissue and in serum were significantly decreased(P<0.01),and the concentrations of IL-4 were significantly increased in the acupuncture group compared with those in the model group at D7 time point(P<0.01);the concentrations of IFN-γwere significantly increased(P<0.01),and the concentrations of IL-4 were significantly decreased in the acupuncture plus miR-34a-5p agomir group compared with those in the acupuncture group and in the acupuncture plus agomir-NC(P<0.01).Conclusion:Needling DU20-through-GB7 can reduce the degree of brain edema,inhibit the release of proinflammatory factor of IFN-γand promote the release of anti-inf
