主动脉内球囊反搏治疗体外循环后泵衰竭室性心律失常的临床效果分析

摘　　要：目的探讨主动脉内球囊反搏(IABP)治疗体外循环后泵衰竭室性心律失常的临床效果。方法66例体外循环后泵衰竭室性心律失常患者,按照随机数字表法分为对照组和观察组,每组33例。对照组给予利多卡因抗心律失常治疗,观察组给予IABP治疗。对比两组临床疗效、治疗前后血流动力学指标。结果观察组治疗总有效率为96.97%,高于对照组的75.76%,差异具有统计学意义(P<0.05);治疗24h后,观察组收缩压(SBP)、舒张压(DBP)、心率分别为(112.63±9.85)mmHg(1mm Hg=0.133kPa)、(80.36±5.41)mmHg、(82.96±6.78)次/min,低于对照组的(120.44±9.97)mmHg、(85.47±5.56)mmHg、(91.35±7.24)次/min,差异有统计学意义(P<0.05)。结论IABP治疗体外循环后泵衰竭室性心律失常的效果更佳,可减慢患者心率,稳定血流动力学。Objective To discuss the clinical effect of intra-aortic balloon pump(IABP)on ventricular arrhythmias after pump failure of cardiopulmonary bypass.Methods A total of 66 patients with ventricular arrhythmias after pump failure of cardiopulmonary bypass were divided into control group and observation group according to the random numerical table,with 33 cases in each group.The control group was treated with lidocaine for antiarrhythmia,and the observation group was treated with IABP.The clinical efficacy and hemodynamic indexes before and after treatment were compared between the two groups.Results The total effective rate of treatment in the observation group was 96.97%,which was higher than 75.76%in the control group,and the difference was statistically significant(P<0.05).After 24h of treatment,the systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate of the observation group were(112.63±9.85)mmHg(1 mm Hg=0.133kPa),(80.36±5.41)mmHg and(82.96±6.78)beats/min,which were lower than(120.44±9.97)mmHg,(85.47±5.56)mmHg and(91.35±7.24)times/min of the control group,and the differences were statistically significant(P<0.05).Conclusion IABP is more effective in the treatment of ventricular arrhythmia after pump failure of cardiopulmonary bypass,which can slow the heart rate and stabilize hemodynamics.
