B超导引下经皮经肝胆囊穿刺造瘘术治疗急性胆囊炎的效果评价

摘　　要：目的研究经B超引导下经皮经肝胆囊穿刺造瘘术治疗急性胆囊炎的效果。方法72例急性胆囊炎患者,随机分为观察组和对照组,各36例。对照组通过常规的腹腔镜下胆囊切除术治疗,观察组通过B超引导下经皮经肝胆囊穿刺造瘘术治疗。比较两组临床疗效、恢复指标。结果观察组治疗总有效率94.44%高于对照组的75.00%,差异有统计学意义(P<0.05)。观察组术中出血量(54.06±4.62)ml少于对照组的(66.47±5.46)ml,手术时间(63.46±10.35)min、术后住院时间(6.67±2.16)d短于对照组的(94.38±12.36)min、(10.34±3.06)d,差异有统计学意义(P<0.05)。结论临床通过B超引导下经皮经肝胆囊穿刺造瘘术治疗急性胆囊炎的效果显著,可促进患者的恢复,值得推荐。Objective To study the effect of percutaneous transhepatic cholecystostomy guided by B-ultrasound in the treatment of acute cholecystitis.Methods A total of 72 patients with acute cholecystitis were randomly divided into the observation group and the control group,with 36 cases in each group.The control group was treated by conventional laparoscopic cholecystectomy,and the observation group was treated by percutaneous transhepatic cholecystostomy under the guidance of B-ultrasound.The clinical efficacy and recovery indexes of the two groups were compared.Results The total effective rate of the observation group was 94.44%,which was higher than 75.00%of the control group,and the difference was statistically significant(P<0.05).The amount of intraoperative bleeding in the observation group was(54.06±4.62)ml,which was less than(66.47±5.46)ml in the control group;the operation time(63.46±10.35)min and postoperative hospitalization time(6.67±2.16)d in the observation group were shorter than(94.38±12.36)min and(10.34±3.06)d in the control group;the differences were statistically significant(P<0.05).Conclusion Clinical treatment of acute cholecystitis by B ultrasound-guided percutaneous transhepatic cholecystostomy is highly effective and can promote patient recovery,which is worthy of recommendation.
