超脉冲CO_(2)点阵激光联合他克莫司软膏治疗寻常型白癜风的效果及对血清IL-17、IL-23水平的影响

摘　　要：目的研究超脉冲二氧化碳(CO_(2))点阵激光联合他克莫司软膏治疗寻常型白癜风的效果及对血清白细胞介素-17(IL-17)、白细胞介素-23(IL-23)水平的影响。方法回顾性分析2019年3月至2020年3月我院皮肤科收治的100例寻常型白癜风患者的临床资料,按照治疗方式不同将其分为对照组和观察组,各50例。对照组采用他克莫司软膏治疗,观察组在对照组基础上联合超脉冲CO_(2)点阵激光治疗。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原4阳性/白细胞分化抗原8阳性(CD4^(+)/CD8^(+))高于对照组(P<0.05)。治疗后,观察组的IL-17、IL-23水平低于对照组(P<0.05)。治疗后,观察组的白斑面积小于对照组(P<0.05)。治疗后,观察组的皮肤病生活质量指数(DLQI)评分高于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论超脉冲CO_(2)点阵激光联合他克莫司软膏治疗寻常型白癜风的效果显著,可有效降低IL-17、IL-23水平,值得临床推广。Objective To study the effect of super-pulsed carbon dioxide(CO_(2))fractional laser combined with tacrolimus ointment in the treatment of vitiligo vulgaris and its influences on serum interleukin-17(IL-17)and interleukin-23(IL-23)levels.Methods The clinical data of 100 patients with vitiligo vulgaris admitted in the dermatology department of our hospital from March 2019 to March 2020 were retrospectively analyzed.According to different treatment methods,the patients were divided into control group and observation group,with 50 cases in each group.The control group was treated with tacrolimus ointment,and the observation group was treated with super-pulsed CO_(2)fractional laser on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the cluster of differentiation 3 positive(CD3^(+)),cluster of differentiation 4 positive(CD4^(+))and cluster of differentiation 4 positive/cluster of differentiation 8 positive(CD4^(+)/CD8^(+))in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of IL-17 and IL-23 in the observation group were lower than those in the control group(P<0.05).After treatment,the white spot area in the observation group was smaller than that in the control group(P<0.05).After treatment,the Dermatology Life Quality Index(DLQI)score in the observation group was higher than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Super-pulsed CO_(2)fractional laser combined with tacrolimus ointment in the treatment of vitiligo vulgaris has a significant effect,it can effectively reduce the levels of IL-17 and IL-23,which is worthy of clinical promotion.
