摘 要:目的 研究孕中期孕妇25(OH)D水平与血糖水平的相关性。方法 选取2018年1—12月于江苏省扬州市妇幼保健院常规建卡产检的妊娠期妇女共1 742名,经筛选共对756名孕妇进行跟踪随访。于孕妇首次产检时了解一般情况,于妊娠18~20周测孕妇血清25(OH)D水平,于妊娠24~28周行口服葡萄糖耐量试验。根据孕妇血清25(OH)D水平,将孕妇分为维生素D缺乏组[180例,血清25(OH)D<50 nmol/L]和维生素D正常组[576例,血清25(OH)D≥50 nmol/L]。对比两组受检者血糖水平差异,并分析妊娠期妇女维生素D水平与血糖水平的相关性。结果 维生素D缺乏组的空腹血糖为(4.59±0.53)mmol/L高于维生素D正常组的空腹血糖(4.48±0.48)mmol/L,差异有统计学意义(t=2.387,P<0.05);25(OH)D水平与空腹血糖、餐后2 h血糖呈显著负相关关系(P<0.05)。结论 低水平25(OH)D的孕妇血糖水平偏高,且25(OH)D水平与空腹血糖、餐后2 h血糖水平有相关性。Objective To study the correlation between vitamin D and blood glucose levels in pregnant women with gestational diabetes. Methods From January to December 2018, a total of 1 742 pregnant women from Yangzhou Maternal and Child Health Hospital in Jiangsu Province for routine card birth examination, and 756 pregnant women were followed up after screening. The general situation of pregnant women was known at the first antenatal examination, the serum 25(OH) D level of pregnant women was measured at 18-20 weeks of gestation, and the oral glucose tolerance test was performed at 24-28 weeks of gestation. According to the serum 25(OH) D level of pregnant women, pregnant women were divided into vitamin D deficiency group [180 cases, serum 25(OH) D<50 nmol/L] and vitamin D normal group [576 cases, serum 25(OH) D≥50 nmol/L]. The difference of blood glucose level between the two groups was compared, and the correlation between vitamin D level and blood glucose level in pregnant women was analyzed. Results The fasting blood glucose in vitamin D deficiency group was(4.59±0.53) mmol/L higher than that in vitamin D normal group(4.48±0.48) mmol/L, the difference was statistically significant(t=2.387, P<0.05). The level of 25(OH) D was negatively correlated with fasting blood glucose and 2 hour postprandial blood glucose(P<0.05). Conclusion The blood glucose level of low level 25(OH) D pregnant women was higher, and 25(OH) D level was correlated with fasting blood glucose and 2 h postprandial blood glucose.
