扶正解毒活血方治疗带状疱疹急性期41例临床观察

摘　　要：目的观察扶正解毒活血方治疗带状疱疹(HZ)急性期的临床疗效及对带状疱疹后神经痛(PHN)的预防作用。方法将81例带状疱疹急性期患者采用随机数字表法分为2组,对照组40例采用西医常规治疗,治疗组41例采用扶正解毒活血方治疗。连续治疗2周后,比较2组的临床疗效,疱疹的止疱时间、结痂时间、脱痂时间,不良反应发生情况,治疗前和治疗3,7,14 d后视觉模拟评分法(VAS)评分,以及治疗后3个月PHN的发生率。结果与对照组比较,治疗组疱疹的止疱时间、结痂时间、脱痂时间均明显缩短,差异有统计学意义(P<0.05);治疗3,7,14 d后,2组VAS评分均明显降低,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组降低更明显,与对照组治疗后同期比较差异均有统计学意义(P<0.05);治疗组PHN发生率(2.44%,1/41)明显低于对照组(22.50%,9/40),2组比较差异有统计学意义(P<0.05);治疗组总有效率为100.00%(41/41),对照组为85.00%(34/40),2组比较差异有统计学意义(P<0.05);2组均未发生明显不良反应。结论采用扶正解毒活血方治疗带状疱疹急性期患者,可迅速缓解神经痛,缩短疱疹愈合时间,有效防止PHN的发生,疗效显著,且无明显不良反应,值得临床推广应用。Objective To observe the clinical efficacy of Fuzheng Jiedu Huoxue Fang(扶正解毒活血方)in the treatment of herpes zoster(HZ)in acute phase and its preventive effect on postherpetic neuralgia(PHN).Methods 81 patients with HZ in acute phase were divided into treatment group and control group using the random number table.40 patients in the control group were treated with conventional Western medicine,while 41 patients in the treatment group were treated with Fuzheng Jiedu Huoxue Fang.After 2 weeks of continuous treatment,the clinical efficacy,stopping time of herpes,scab formation time,scab removal time,adverse reactions,visual analogue scale(VAS)scores before treatment and after treatment for 3,7,14 d,and the incidence of PHN 3 months after treatment were compared between the two groups.Results Compared with the control group,the blister stopping time,scab formation time and scab removal time of herpes in the treatment group were shortened significantly,and the difference was statistically significant(P<0.05).After treatment for 3,7 and 14 d,the VAS scores of the two groups were decreased significantly,and the difference was statistically significant compared with the same group before treatment(P<0.05);the decrease in the treatment group was more obvious,and the difference was statistically significant compared with the control group after treatment(P<0.05).The incidence of PHN in the treatment group(2.44%,1/41)was significantly lower than that in the control group(22.50%,9/40)(P<0.05).The total effective rate was 100.00%(41/41)in the treatment group and 85.00%(34/40)in the control group,the difference being significant(P<0.05).No obvious adverse reactions occurred in both groups.Conclusion It can relieve neuralgia markedly,shorten the healing time of herpes and prevent the occurrence of PHN effectively to use Fuzheng Jiedu Huoxue Fang in the treatment of patients with HZ in acute phase.It deserves clinical promotion and application with significant effect and no obvious adverse reaction.
