摘 要:目的:探讨代谢综合征组分及其组合方式与脑卒中发病的关联。方法:采用前瞻性队列研究比较脑卒中发病与未发病人群之间的人口学信息、代谢综合征相关指标及阳性组分数量差异,采用多因素logistic回归分析模型分析代谢综合征组分及其组合方式与脑卒中发病的关联。结果:单因素logistic回归分析结果显示,代谢综合征各组分中,除甘油三酯升高外,其余4个组分均为脑卒中发病的危险因素;多因素logistic回归分析结果显示,腹型肥胖+血压升高+血糖升高为危险系数最高的组分组合方式。结论:代谢综合征为脑卒中发病的危险因素,其发病风险随代谢综合征阳性组分数量的增加而增加,腹型肥胖+血压升高+血糖升高为脑卒中发病危险性最高的组合方式。在脑卒中疾病防治工作中,应加强对人群糖尿病、高血压及肥胖的早期干预。OBJECTIVE To investigate the association of metabolic syndrome(MS)components and their combinations with stroke.METHODS A prospective cohort study was conducted to compare the differences in demographic information,MS-related indexes,and the number of positive components between stroke patients and those without stroke.Logistic regression was performed to analyze the association of MS components and their combinations with stroke.RESULTS Univariate logistic analysis showed that among MS components,except for elevated TG,the other four components were risk factors for stroke.Multivariate logistic analysis suggested that the combination of abdominal obesity,elevated blood pressure,and elevated blood glucose was the highest risk component combination.CONCLUSION MS is a risk factor for stroke which risk increases with the number of MS-positive components.Moreover,elevated blood sugar,elevated blood pressure,and abdominal obesity are the combinations with the highest risk of stroke.Therefore,early intervention on diabetes,hypertension,and obesity in the population should be strengthened for stroke.
