摘　　要：目的通过探讨总结胎儿Dandy-Walker综合征MRI影像表现特征并分析其差异,以提高对Dandy-Walker综合征的产前影像诊断水平。方法通过查阅相关文献,分析乐山市妇幼保健院2020年4月至2021年12月经证实的14例Dandy-Walker综合征的临床病例及MRI影像资料。结果本组14例病例中,5例Dandy-walker畸形MRI影像特征为重度小脑蚓部结构不完整、缺损面积大,小脑半球分离,第四脑室扩大、后颅窝池增宽,小脑幕明显抬高;3例Dandy-walker变异型者MRI影像特征为轻度小脑下蚓部结构不全、缺损面积小,伴有轻度第四脑室扩张、顶角变钝,小脑蚓部轻度上旋,后颅窝池扩大及小脑幕上抬不明显,小脑半球大小、形态结构正常;6例单纯后颅窝池增宽者的MRI影像仅表现为小脑延髓池增宽(>10mm),小脑半球及小脑蚓部发育正常,四脑室形态大小未见异常,小脑幕位置正常。结论针对性胎儿MRI对于产前明确诊断有重要价值,MRI可清晰、完整地显示胎儿后颅窝解剖结构,客观、准确的评价小脑蚓部发育情况,精准的鉴别Dandy-Walker畸形各亚型及伴随改变。Objective By analyzing the imaging characteristics and summaring differences of fetal Dandy-Walker syndrome to improve the prenatal imaging diagnosis of Dandy-Walker syndrome.Methods In addition to by looking up the relevant literature,a retrospective review was conducted of clinical records and MRI imaging data of 14 cases with Dandy-Walker syndrome admitted of our hospital between April 2020 to December 2021.Results Among the 14 cases in this group,MRI images of five Dandy-walker malformation cases were characterized by a severe incomplete cerebellar vermis structure,large defect area,cerebellar hemisphere separation,fourth ventricle enlargement,posterior cranial fossadilatation,beside cerebellar curtain significantelevation;MRI imaging of three Dandy-walker variants cases was characterized by mild hypocerebellar vermis structural insufficiency and small defect area,Withmidly fourth ventricle dilatation andapical angleblunted,The cerebellar vermis was slightly upspin,The enlargement of the posterior cranial fossa and the cerebellar curtainlifting were not obvious,cerebellar hemisphere’s size and morphological structure are normal;MRI imaging of six Mega cistern magna cases showedonly enlargement of the cisterna cerebellomedullaris(>10mm),The cerebellar hemispheres and the vermis developed as normal,No abnormalities was observed in the fourth ventricle and cerebellar curtain position was not elevated.Conclusion Pertinence fetal MRI has important value in prenatal diagnosis.Beside show the anatomy of the fetal posterior fossa clearly and completely,MRI can judge the development of the cerebellar vermisobjectively and accurately,identificate the various subtypes of the Dandy-Walker malformation precisely and the accompanying changes.
