摘　　要：目的 研究蛇药敷贴联合循证护理干预对毒蛇咬伤患者功能康复影响及并发症的预防效果。方法 收集2017年2月—2019年6月在浙江省丽水市中心医院就诊的89例毒蛇咬伤患者的临床资料,依据入院先后顺序分为对照组(n=43)和观察组(n=46)。对照组采用蛇药敷贴联合常规护理,观察组采用蛇药敷贴联合循证护理。观察并比较两组临床疗效,干预前后视觉模拟评分法(VAS)、患肢周径、生命体征、肢体活动障碍评分、并发症发生情况。结果 观察组总有效率高于对照组(P<0.05)。干预后,两组VAS评分、患肢周径、平均动脉压、呼吸、脉搏、体温、肢体活动障碍评分均较干预前下降,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组总并发症发生率低于对照组(P<0.05)。结论 蛇药敷贴联合循证护理干预能够提高毒蛇咬伤患者的治愈率,减轻创面疼痛和肿胀,促进功能康复,减少并发症的发生。Objective To study the effects of snake medicine application combined with evidence-based nursing intervention on functional rehabilitation of patients bitten by poisonous snakes and preventive effect of complications.Methods The clinical data of 89 patients with venomous snakebite who were treated in Lishui Central Hospital of Zhejiang Province from February 2017 to June 2019 were collected and divided into control group(n=43) and study group(n=46) according to the order of admission.The control group was given snake medicine application combined with routine nursing,and the study group was given snake medicine application combined with evidence-based nursing.The clinical efficacy,visual analog scale(VAS),limb circumference,vital signs,limb movement disorder score,and complications before and after intervention were observed and compared between the two groups.Results The total effective rate of the study group was 89.13%,significantly higher than that of the control group(69.77% P<0.05),Before intervention,there were no significant differences inthe values of VAS score,peripheral diameter of affected limb,mean arterial pressure,respiration rate,pulse rate,body temperature and limb dyskinesia score between these groups(all P<0.05).After intervention,all these values significantly decreased in these 2 groups(all P<0.05),and the values of the research group were all significantly lower than those of the control group(all P<0.05).The incidence rate of complication of the research group was 6.52%,significantly lower than that in the control group(20.93%,P<0.05).Conclusion Snake medicine application combined with evidence-based nursing intervention can improve the cure rate of snake bite patients,reduce wound pain and swelling,promote functional recovery and reduce the occurrence of complications.
