2011-2020年镇江市登记新发肺结核患者流行特征分析

摘　　要：目的了解2011-2020年镇江市新发肺结核流行状况及趋势。方法通过结核病信息管理系统收集2011-2020年镇江市登记肺结核病人的年龄、性别、病原学分类、职业等资料,对流行病学特征进行描述性分析。结果2011-2020年均登记率为42.83/10万,从2011年的57.88/10万下降到2020年的28.70/10万,年均递减率为5.60%,差异有统计学意义(χ^(2)_(趋势)=2129.375,P<0.001)。男性患者登记率为63.63/10万,女性患者登记率为22.42/10万,差异有统计学意义(χ^(2)=3150.330,P<0.001)。2018-2020年病原学阳性率呈上升趋势。肺结核病患者年龄分布集中在45岁以上年龄段,累计登记数占登记总数的69.11%。职业分布以农民为主,占58.16%。结论镇江市登记新发肺结核疫情呈下降趋势,但需加强老年人、农民的结核病防控工作。Objective To understand the epidemiological characteristics of newly registered pulmonary tuberculosis(PTB)in Zhenjiang city from 2011 to 2020.Methods The data of newly registered PTB cases in Zhenjiang city,including age,gender,pathogen classification,occupation etc.,was collected from the National Infectious Disease Reporting and Management System between 2011 to 2020 while the epidemiological characteristics of cases were described by descriptive analysis method.Results The average annual registration rate throughout 2011-2020 was 42.83 per 100000,with a significant annual decline of 5.60%(from 57.88 per 100000 in 2011 to 28.70 per 100000 in 2020,χ^(2)_(trend)=2129.375,P<0.001).There was significant difference in mean annual registration rate across gender,which was 63.63 per 100000 for men and 22.42 per 100000 for the women,respectively(χ^(2)=3150.330,P<0.001).The pathogen positivity among registered patients increased from 2018 to 2020.The PTB patients aged 45 years and older accounted for 69.11%and farmers accounted for 58.16%.Conclusions From 2011 to 2020,the PTB reported incidence in Zhenjiang showed a steady downward trend.More attention should be paid to the prevention and control of PTB in key occupations and age groups such as farmers and elderly people.
