游绍伟运用“通腑法”治疗大肠积滞型腰痛经验

摘　　要：腰痛是指因各种原因导致腰部或气血运行不畅,或失于濡养,引起的以腰部一侧或两侧发生疼痛为主要症状的疾病。中医学认为腰为肾之府,多从肾论治腰痛。游绍伟主任从医20余载,总结多年临证经验,结合古籍经典,基于“脾为之使,胃为之市”理论,认为燥屎积聚肠腑,腰部局部气血运行不畅,也会导致腰痛,并结合胃肠与腰椎的经络、解剖生理、神经调节的关系,运用“通腑法”治疗大肠积滞引起的腰痛疗效较好。本文主要总结游绍伟从脾胃肠论治腰痛的临证经验,并附验案一则,以期为治疗腰痛提供参考。Low back pain(LBP)is a disease with the main symptoms of pain on one side or both sides of the low back because of poor qi and blood circulation or loss of nourishment in the low back.Traditional Chinese medicine(TCM)believes that the low back is the house of the kidney,and LBP is mostly treated from the kidney.Chief physician You Shaowei has been engaged in clinical work for more than 20 years,with years of clinical experience accumulated.In combination with ancient medical classics and based on the theory of"the spleen acting as the envoy and the stomach serving as a market over the diaphragm",he believes that the dry stool accumulation in the large intestine facilitates poor qi and blood circulation in the low back,leading to LBP.Based on the relationship of stomach/intestine and lumbar vertebra meridians and collaterals,anatomical physiology,and nerve regulation,he treats LBP induced by the accumulation in the large intestine by"dredging Fu-organs",with good clinical efficacy.This study mainly summarized the clinical experience of You Shaowei in the treatment of LBP from the spleen and stomach and attached a medical case to provide references for the treatment of LBP.
