益气升阳法治疗脾虚湿浊型盆腔炎性疾病后遗症的体会

摘　　要：盆腔炎性疾病(PID)为妇科常见疾病,反复发作、迁延难愈,可同时继发慢性盆腔痛、不孕、异位妊娠等盆腔炎性疾病后遗症(SPID),严重影响女性的生活质量。中医认为,脾虚为SPID的主要内因之一,脾虚则阳气下陷,脾失健运则痰浊内生,下注胞宫,脾虚气滞、寒湿内结则可生瘀,SPID的发生、发展与脾脏功能失调而致气滞、寒湿密不可分。益气升阳法可以健脾益气、升阳举陷、化痰行气,笔者运用益气升阳除湿之法治疗脾虚湿浊型SPID,并在益气升阳思路指导下以升阳除湿防风汤为常用基础方,临床治疗效果较佳。Pelvic inflammatory disease(PID)is a common refractory gynecological disease with recurrent attacks and prolonged course and is accompanied by sequelae of pelvic inflammatory disease(SPID),such as chronic pelvic pain,infertility,and ectopic pregnancy,which seriously affects the quality of life of women.Traditional Chinese medicine(TCM)believes that spleen deficiency is one of the main internal causes of SPID.Because of yang sinking by spleen deficiency,the dysfunction of the spleen in transportation can induce the internal stagnation of phlegm turbidity,which pours down to the uterus with appendages.Moreover,spleen deficiency and qi stagnation,and the cold-dampness internal combination can result in blood stasis.The occurrence and development of SPID are closely related to qi stagnation and cold-dampness caused by spleen dysfunction.The method of replenishing qi and raising yang can strengthen the spleen and tonify qi,elevate yang and raise the drooping,resolve phlegm and promote qi movement.SPID with spleen deficiency and dampness turbidity can be treated by nourishing qi,raising yang,and removing dampness.Under the guidance of the idea of replenishing qi and raising yang,Shengyang Chushi Fangfeng Decoction can serve as a basic prescription,with good clinical efficacy.
