2005—2020年北京市通州区女性乳腺癌及生殖系统主要恶性肿瘤死亡趋势分析

摘　　要：目的分析2005—2020年北京市通州区女性乳腺癌及宫颈癌、子宫体癌、卵巢癌3种生殖系统主要恶性肿瘤死亡水平及变化趋势,为女性恶性肿瘤防控工作提供参考依据。方法利用全国人口死亡信息登记管理系统,导出2005—2020年北京市通州区女性乳腺癌、宫颈癌、子宫体癌、卵巢癌死亡病例数据,计算4种恶性肿瘤死亡率、中国人口标化死亡率(中标率)、累积死亡率(0~74岁)(累积率)、截缩死亡率(35~64岁)(截缩率)和过早死亡寿命损失年(years of life lost,YLL)。应用JoinPoint软件分析4种恶性肿瘤死亡率和YLL变化趋势,计算年度变化百分比(annual percent of change,APC)。结果2005—2020年北京市通州区女性乳腺癌死亡559例,死亡率为9.84/10万,中标率为7.11/10万,累积率为0.60%,截缩率为12.12%;宫颈癌死亡139例,死亡率为2.45/10万,中标率为1.89/10万,累积率为0.15%,截缩率为3.25%;子宫体癌死亡129例,死亡率为2.27/10万,中标率为1.56/10万,累积率为0.14%,截缩率为2.12%;卵巢癌死亡248例,死亡率为4.37/10万,中标率为3.16/10万,累积率为0.29%,截缩率为5.08%。2020年较2005年乳腺癌、宫颈癌、子宫体癌、卵巢癌YLL均呈增加趋势。2005—2020年乳腺癌、卵巢癌死亡率APC均呈上升趋势(均P<0.05),卵巢癌中标率、累积率、截缩率APC呈上升特征(均P<0.05)。结论2005—2020年北京市通州区女性乳腺癌及生殖系统主要恶性肿瘤死亡率变化平稳,YLL呈增加特征。宫颈癌死亡年龄呈年轻化,卵巢癌疾病负担呈上升趋势,应重点关注并加强防控。Objective To analyze the mortality and change trend of breast cancer and main malignant tumors of the reproductive system,including cervical cancer,corpus uteri cancer and ovarian cancer,in females in Tongzhou District of Beijing from 2005-2020,provide a reference for the prevention and control of female malignant tumors.Methods Based on the national population death information registration management system,the data of death cases from breast cancer,cervical cancer,corpus uteri cancer and ovarian cancer in females in Tongzhou District of Beijing from 2005-2020 were derived,to calculate the mortality rate of four malignant tumors,standardized mortality rate of Chinese population,cumulative mortality rate(0-74 years old),truncated mortality rate(35-64 years old)and years of life lost(YLL).The joinpoint software was used to analyze the mortality and YLL change trend of four malignant tumors,and calculate the annual percentage of change(APC).Results From 2005 to 2020,there were 559 female cases died of breast cancer in Tongzhou District of Beijing,the mortality rate was 9.84/100000,the standardized mortality rate was 7.11/100000,and the cumulative mortality rate was 0.60%,and truncated mortality rate was 12.12%.There were 139 cases died of cervical cancer,themortality rate was 2.45/100000,the standardized mortality rate was 1.89/100000,the cumulative mortality rate was 0.15%,and the truncated mortality rate was 3.25%.There were 129 cases died of corpus uteri cancer,the mortality rate was 2.27/100000,the standardized mortality rate was 1.56/100000,the cumulative mortality rate was 0.14%,and the truncated mortality rate was 2.12%.There were 248 cases died of ovarian cancer,the mortality rate was 4.37/100000,the standardized mortality rate was 3.16/100000,the cumulative mortality rate was 0.29%,and the truncated mortality rate was 5.08%.In 2020,the YLL of breast cancer,cervical cancer,corpus uteri cancer and ovarian cancer had rising trends as compared with 2005.From 2005 to 2020,the APC of breast cancer and ovarian c
