摘 要:目的测定1,25二羟维生素D3[1,25 dihydroxyvitamin D3,1,25(OH)2D3]及其可溶性受体(vitamin D receptor,VDR)在布鲁氏菌病患者血清中的水平,探讨其在该病中的意义。方法选取2019年11月至2020年11月青海大学附属医院确诊的布鲁氏菌病患者21例(布病组),将布病组又分为骨关节受累组(14例)和无骨关节受累组(7例);选取同期我院门诊体检健康者23例作为对照组。采用酶联免疫吸附法(enzyme linked immunosorbent assay,ELISA)测定布病组及对照组血清中1,25(OH)2D3及VDR的水平,分析其在布鲁氏菌病中的表达变化。结果布病组患者血清中1,25(OH)2D3及VDR水平均显著低于对照组,差异均有统计学意义{[254.84(177.90,286.72)]pg/mL比[286.05(272.15,304.67)]pg/mL,[2.28(1.81,2.91)]ng/mL比[4.07(1.99,6.77)]ng/mL,z值分别为-2.46,-2.19,P值均<0.05};与骨关节受累组相比,无骨关节受累组患者血清1,25(OH)2D3及VDR水平都有所升高,但组间差异均无统计学意义(P>0.05)。结论布鲁氏菌病组血清中1,25(OH)2D3及VDR表达水平下降,推测维生素D信号通路可能参与了布鲁氏菌病的病理改变。Objective To determine the level of 1,25 dihydroxyvitamin D3[1,25(OH)2D3]and the soluble vitamin D receptor(VDR)in the serum of patients with Brucellosis,and to explore its significance in the disease.Methods Twenty one Brucellosis patients(Brucellosis group)diagnosed in the Affiliated Hospital of Qinghai University from November 2019 to November 2020 were selected.The Brucellosis group was divided into bone and joint involvement group(14 cases)and no bone and joint involvement group(7 cases).Twenty-three healthy people who underwent physical examination in our hospital during the same period were selected as the normal control group for this study.The levels of 1,25(OH)2D3 and VDR in serum of brucellosis group and control group were determined by enzyme linked immunosorbent assay(ELISA),and the changes of their expression in brucellosis were analyzed.Results The serum levels of 1,25(OH)2D3 and VDR in the Brucellosis group were significantly lower than those in the control group{[254.84(177.90,286.72)]pg/mL vs[286.05(272.15,304.67)]pg/mL,[2.28(1.81,2.91)]ng/mL vs[4.07(1.99,6.77)],Z values were-2.46 and-2.19 respectively,both P values<0.05}.Compared with the bone and joint involvement group,the serum levels of 1,25(OH)2D3 and VDR in the non bone and joint involvement group were increased,but there was no significant difference between the groups(P>0.05).Conclusion The expression levels of 1,25(OH)2D3 and VDR in serum of Brucellosis group decreased,suggesting that vitamin D signaling pathway may be involved in the pathological changes of Brucellosis.
