地屈孕酮联合米非司酮在围绝经期异常子宫出血患者中的应用效果

摘　　要：目的:探究地屈孕酮联合米非司酮治疗对围绝经期异常子宫出血患者子宫内膜厚度及性激素水平的影响。方法:选取60例2019年1月至2021年7月宜黄县妇幼保健和计划生育服务中心收治的围绝经期异常子宫出血患者,按照随机数字表法分为观察组30例,对照组30例。两组均给予抗感染、抗贫血常规治疗,在此基础上,观察组给予米非司酮片、地屈孕酮片治疗,而对照组仅给予米非司酮片治疗,均治疗21 d。比较治疗21 d后的临床疗效,治疗期间不良反应,治疗前及治疗21 d后的月经量、子宫内膜厚度、血浆血红蛋白(HGB)及性激素水平。结果:治疗后,观察组总有效率高于对照组(P<0.05);相较治疗前,两组月经量、子宫内膜厚度均减少,且观察组优于对照组(P<0.05);血浆HGB水平升高,且观察组高于对照组(P<0.05);血清促性腺激素(FSH)、黄体生成素(LH)、孕酮(P)水平降低,且观察组均低于对照组(均P<0.05);不良反应总发生率观察组低于对照组,差异有统计学意义(P<0.05)。结论:地屈孕酮联合米非司酮治疗围绝经期异常子宫出血可有效改善患者的临床症状,减少月经量及子宫内膜厚度,调节血红蛋白及性激素水平,减少不良反应的发生,整体安全性较高。Objective:To investigate the effect of dydrogesterone combined with mifepristone on endometrial thickness and sex hormone levels in perimenopausal patients with abnormal uterine bleeding.Methods:Select 60 cases of perimenopausal abnormal uterine bleeding patients admitted to Yihuang County Maternal and Child Health and Family Planning Service Center from January 2019 to July 2021,and divide them into observation group(30 cases)and control group(30 cases)according to random number table.Both groups were given conventional anti-infection and anti-anemia treatment.On this basis,the observation group was given mifepristone tablets and dydrogesterone tablets,while the control group was only given mifepristone tablets,all for 21 days.The clinical efficacy,adverse reactions during the treatment,menstrual volume,endometrial thickness,plasma hemoglobin(HGB)and sex hormone levels before and after 21 days of treatment were compared.Results:After treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).Compared with before treatment,the menstrual volume and endometrial thickness of the two groups were reduced,and the observation group was better than the control group(P<0.05).The plasma HGB level increased,and the observation group was higher than the control group(P<0.05).Serum levels of gonadotropin(FSH),luteinizing hormone(LH)and progesterone(P)decreased,and the observation group were lower than the control group(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Dydrogesterone combined with mifepristone in the treatment of perimenopausal abnormal uterine bleeding can effectively improve the clinical symptoms of patients,reduce menstrual flow and endometrial thickness,regulate the levels of hemoglobin and sex hormones,and reduce the occurrence of adverse reactions.The overall safety is better.
