三种触发扫描技术在下肢动脉CT血管成像中的应用

摘　　要：目的比较经3种触发扫描技术得到的下肢动脉CT血管成像(CTA)图像质量,探讨最适合下肢动脉CTA的扫描方式。方法选取2021年1—9月医院收治的需行CTA检查的疑似下肢动脉疾病患者45例,按照随机数字表法分成A、B、C组,每组15例。3组分别采用腹主动脉自动触发技术、腘动脉手动触发技术和腹主动脉手动触发技术,比较3组的延迟扫描时间和图像质量。结果A组的延迟扫描时间短于B、C组,差异有统计学意义(P<0.05);B、C组的延迟扫描时间比较,差异无统计学意义(P>0.05)。在动脉血管的CT值、动脉分支评分、动脉清晰度评分这3个图像质量指标方面:B、C组的图像CT值及评分在多个下肢血管区域均优于A组,差异有统计学意义(P<0.05)。在静脉干扰评分方面:B组的图像静脉干扰评分高于A、C组,差异有统计学意义(P<0.05)。结论采用腹主动脉手动触发技术能够兼顾患者的个体差异性,可操作性强,能够精准把握扫描时机,且受患者血管闭塞与钙化的影响较小,能够有效提高下肢动脉CTA的图像质量。Objective The image quality of CT angiography(CTA)of lower extremity arteries obtained by three trigger scanning techniques was compared,and the most suitable scanning mode for CTA of lower extremity arteries was explored.Methods 45 patients with suspected lower limb arterial disease who need CTA examination admitted to the hospital from January to September 2021,were selected and divided into 3 groups according to the random number table,with 15 patients in each group.The abdominal aorta automatic trigger technique,popliteal artery manual trigger technique and abdominal aorta manual trigger technique were adopted by group A,B and C respectively,and the delay scanning time and image quality of the three groups were compared.Results The delayed scanning time of group A was shorter than that of group B and group C,with statistically significant difference(P<0.05);There was no significant difference in delayed scanning time between group B and group C(P>0.05).In terms of CT value,arterial branch score and arterial clarity score of arterial vessels,the image CT value and score of Group B and Group C were better than those of Group A in several lower limb vascular regions,with statistically significant difference(P<0.05).In terms of venous interference score,the image venous interference score of group B was higher than that of group A and C,with statistically significant difference(P<0.05).Conclusion With the strong operability and consideration of individual differences of patients,manual trigger technology of abdominal aorta can accurately grasp the scanning time,and is less affected by the occlusion and calcification of patients’blood vessels,which can effectively improve the image quality of CTA of lower limb arteries.
