外周血淋巴细胞的VCS参数与系统性红斑狼疮疾病活动性的相关性

摘　　要：目的探讨系统性红斑狼疮(SLE)患者外周血淋巴细胞的VCS参数[包括淋巴细胞体积(LV)、淋巴细胞电导率(LC)、淋巴细胞光散射性(LS)]与疾病活动性的相关性。方法选取2018年1月至2021年3月河北省沧州中西医结合医院风湿免疫科收治的134例SLE患者作为研究对象,根据系统性红斑狼疮疾病活动指数(SLEDAI)评分将患者分为活动组(83例,SLEDAI评分≥10分)和缓解组(51例,SLEDAI评分<10分),另选取同期的健康体检者60名作为对照组,比较3组的LV、LC、LS及各参数的平均分布宽度(SD)。结果活动组的LV高于缓解组和对照组,差异均有统计学意义(P<0.05);活动组的LC低于对照组,差异有统计学意义(P<0.05);活动组的LV-SD、LC-SD、LS-SD和缓解组的LV-SD、LS-SD均高于对照组,差异有统计学意义(P<0.05)。Pearson检验结果显示,活动组的LV与SLE疾病活动性成正相关(r=0.586,P<0.05);其余参数与SLE疾病活动性均无相关性。ROC分析结果显示,LV诊断活动期SLE的曲线下面积为0.848(95%CI为0.780~0.915),最佳截断值为85.29 fl,此时约登指数为0.688,诊断灵敏度为77.1%,特异度为91.7%。结论外周血淋巴细胞的VCS参数可以反映SLE患者淋巴细胞的形态及内部结构的变化,其中LV与疾病活动性密切相关,对判断SLE患者是否处于活动期具有较好的参考价值。Objective To investigate the correlation between VCS parameters of peripheral blood lymphocytes[including lymphocyte volume(LV),lymphocyte conductivity(LC),lymphocyte light scattering(LS)]and disease activity in patients with systemic lupus erythematosus(SLE).Methods 134 SLE patients admitted to the Rheumatology and Immunology Department of Cangzhou Integrated Traditional and Western Medicine Hospital in Hebei Province from January 2018 to March 2021 were selected as the study subjects.Patients were divided into activity group(83 cases,SLEDAI score≥10 points)and remission group(51cases,SLEDAI score<10 points)according to SLEDAI score.Another 60 health examinees in the same period were selected as the control group,LV,LC,LS and the average distribution width(SD)of each parameter were compared among the three groups.Results The LV of the active group was higher than that of the remission group and the control group(P<0.05);LC in the active group was lower than that in the control group(P<0.05);LV-SD,LC-SD,LS-SD in the active group and LV-SD,LS-SD in the remission group were significantly higher than those in the control group(P<0.05).Pearson test showed that LV in active group was positively correlated with SLE disease activity(r=0.586,P<0.05);Other parameters had no correlation with SLE disease activity.ROC analysis results showed that the area under the curve of SLE in the active phase of LV diagnosis was 0.848(95%CI:0.780~0.915),and the optimal cut-off value was 85.29 fl.At this time,the Jordan index was 0.688,the diagnostic sensitivity was 77.1%,and the specificity was 91.7%.Conclusions The VCS parameters of peripheral blood lymphocytes can reflect the changes in the morphology and internal structure of lymphocytes in SLE patients.LV is closely related to disease activity,which has a good reference value for judging whether SLE patients are in active stage.
