六味地黄汤加味在肾间质纤维化患者中的应用进展

摘　　要：在临床上,慢性肾脏病(CKD)是一种常见疾病,主要由糖尿病、高血压等疾病引起。肾间质纤维化(RIF)可以延缓肾小球硬化、保护肾单位、提高机体免疫功能、减少尿蛋白,其作为CKD发展的病理生理学过程,延迟CKD发病进展,逆转RIF是非常必要的。在中医范畴中,CKD属于肾虚水肿肾虚腰痛。近几年,中医事业的蓬勃发展,六味地黄汤在临床上已经得到广泛应用,并且在临床治疗中范围不断扩大,在慢性肾病综合征患者的治疗中发挥着重要作用,有着多机制、多靶点等优点,配方主要包含坤草、人参、山药、茯苓等。本次研究主要从以下几方面开展:概述现代医学对RIF的认识,即发病机制;概述中医对RIF的认识,即中医病名、病因病机;详细解说六味地黄汤功效与药理,包含对免疫系统的作用、抗肿瘤的作用、对心血管系统的作用、对泌尿生殖系统的作用;讲解临床与实验相关研究;进一步需要改进的地方。通过给予患者六味地黄汤加味之后,治疗效果较为显著。本文展开对RIF患者治疗中适用六味地黄汤治疗的进展进行综述,希望为临床继续研究治疗此疾病的更好方法提供一份参考资料,为患者谋福音。Clinically,chronic kidney disease(CKD)is a common disease,mainly caused by diabetes,hypertension and other diseases.Renal interstitial fibrosis(RIF)can delay glomerulosclerosis,protect nephrons,improve immune function,and reduce urinary protein.As a pathophysiological process of CKD development,it is necessary to delay the progression of CKD and reverse RIF.In the field of traditional Chinese medicine,CKD belongs to kidney deficiency edema and kidney deficiency low back pain.In recent years,with the vigorous development of traditional Chinese medicine,Liuwei Dihuang Decoction has been widely used in clinical practice,and its scope in clinical treatment has continued to expand.It plays an important role in the treatment of patients with chronic nephrotic syndrome.It has multiple mechanisms and multiple targets.Points and other advantages,the formula mainly includes Kuncao,ginseng,yam,Poria and so on.This research is mainly carried out from the following aspects:an overview of modern medicine's understanding of RIF,that is,the pathogenesis;an overview of traditional Chinese medicine's understanding of RIF,that is,the name of TCM disease,etiology and pathogenesis;the role of anti-tumor,the role of the cardiovascular system,the role of the genitourinary system;explain clinical and experimental research;further improvements are needed.After adding Liuwei Dihuang Decoction to the patients,the therapeutic effect was more significant.This article summarizes the progress of applying Liuwei Dihuang Decoction in the treatment of RIF patients,hoping to provide a reference for continued clinical research on better methods for the treatment of this disease,and to seek good news for patients.
