全髋置换术老年患者复苏室苏醒时间影响因素分析及其护理策略

摘　　要：目的分析全髋置换术老年手术患者复苏室苏醒时间的影响因素及护理策略。方法回顾性分析2019年6月至2021年12月于中山市中医院收治的196例行全髋置换术的老年患者临床资料,根据患者术后复苏室苏醒时间分为>60 min组和≤60 min组,比较两组患者术后并发症的发生情况,对两组患者临床资料进行单因素及多因素logistic回归分析,分析老年患者复苏室苏醒时间的影响因素,并制订护理策略。结果196例患者中,麻醉苏醒时间>60 min组38例,≤60 min组158例。麻醉苏醒时间>60 min组患者的恶心呕吐、低氧血症、低血压、术后意识障碍、非人为低体温、深静脉血栓形成、呼吸系统并发症发生率高于≤60 min组,差异有统计学意义(P<0.05)。单因素分析结果显示,两组间性别、年龄、体重指数(BMI)、心率、手术时长、麻醉时长、手术室温度、术中出血量、术中低体温及术后体温下降幅度比较,差异有统计学意义(P<0.05)。多因素logistic回归分析显示,性别(男性)、年龄、手术时长、麻醉时长、术中低体温及术后体温下降幅度为影响全髋置换术老年手术患者复苏室苏醒时间的危险因素,BMI和手术室温度为保护因素(P<0.05)。结论手术室护理过程应充分考虑影响患者麻醉苏醒时间的独立影响因素,缩短手术和麻醉时长,时刻留意手术室温度和患者的体温变化,及时进行调节,以缩短患者苏醒时间,减少术后不良并发症。Objective To analyze the influencing factors and nursing strategies of recovery time in the recovery room of elderly patients undergoing total hip arthroplasty.Methods The clinical data of 196 elderly patients undergoing total hip arthroplasty admitted to Zhongshan Hospital of Traditional Chinese Medicine from December 2019 to December 2021 were retrospectively analyzed.According to the recovery time of postoperative recovery room,the patients were divided into the>60 min group and≤60 min group.The occurrence of postoperative complications was compared between the two groups,and the clinical data of the two groups were analyzed by univariate and multivariate logistic regression analysis,and the influencing factors of the resuscitation time of the elderly patients were analyzed,and the nursing strategy was formulated.Results Among 196 patients,38 cases were in the anesthesia recovery time>60 min group and 158 cases were in the≤60 min group.The incidence of nausea and vomiting,hypoxemia,hypotension,postoperative consciousness disturbance,non-human hypothermia,deep vein thrombosis and respiratory complications in the anesthesia recovery time>60 min group was higher than that in the≤60 min group,and the differences were statistically significant(P<0.05).Univariate analysis showed that there were significant differences in gender,age,body mass index(BMI),heart rate,operation duration,anesthesia duration,operating room temperature,intraoperative blood loss,intraoperative hypothermia and postoperative temperature drop between the two groups(P<0.05).Multivariate logistic regression analysis showed that gender(male),age,duration of operation,duration of anesthesia,intraoperative hypothermia and postoperative temperature drop were risk factors affecting recovery time in resuscitation room of elderly patients undergoing total hip replacement,and BMI and operating room temperature were protective factors(P<0.05).Conclusion The operating room nursing process should fully consider the independent influencing factors of a
