摘  要:目的探讨Revolution CT能谱扫描联合自适应迭代重建(ASIR-V)前置迭代算法在肺结节诊断中的优势。方法收集郑州大学第一附属医院2019年12月至2022年3月病理证实的100例恶性孤立性肺结节患者,分为两组,各50例,一组经Revolution CT能谱扫描联合ASIR-V前置迭代算法重建,即低剂量能谱扫描,一组行常规能谱扫描。测量结节CT值及标准差(SD),记录剂量容积指数(CTDIvol)及扫描长度,计算结节信噪比(SNR)、对比度噪声比(CNR)、剂量长度乘积(DLP)及有效剂量(ED),并对结节的显示行主观评分。测量结节双期碘水基值及40 keV下CT值并计算能谱曲线斜率。结果(1)两组间静脉期CT值比较差异有统计学意义(P<0.05)。动脉期CT值,双期SNR、CNR值、碘基值、水基值、40 keV下CT值及能谱曲线斜率比较差异无统计学意义(P>0.05)。(2)低剂量能谱CT扫描肺结节CTDIvol、DLP、ED值均较常规能谱CT减小,差异有统计学意义(P<0.01)。(3)两组肺结节主观评分均在3分以上,差异无统计学意义(P>0.05)。结论Revolution CT能谱扫描联合ASIR-V前置迭代算法较常规能谱扫描在不影响肺结节的显示与能谱参数的测量前提下,辐射剂量明显降低,具有明显优势。Objective To investigate the feasibility of Revolution CT spectral scanning combined with adaptive statistical iterative reconstruction V(ASIR-V)preiteration algorithm in the diagnosis of pulmonary nodules.Methods A total of 100 patients with malignant solitary pulmonary nodules confirmed by pathology in the First Affiliated Hospital of Zhengzhou University from December 2019 to March 2022 were collected and divided into 2 groups,with 50 cases in each group.One group was underwent by Revolution CT low-dose spectral scanning combined with ASIR-V preiteration algorithm,and the other group underwent conventional spectral scanning.The CT value and standard deviation(SD)of nodules were measured,and the dose volume index(CTDIvol)and scan length were recorded.The nodule signal noise ratio(SNR),contrast to noise ratio(CNR),dose length product(DLP)and effective dose(ED)were calculated,and the nodule display was evaluated by subjective score.The biphasic iodine water base value and CT value at 40 keV were measured and the slope of the spectral curve was calculated.Results(1)There was significant difference in CT value of venous phase between the two groups(P<0.05).There was no significant difference in the CT value of arterial phase,SNR value,CNR value,iodine base value,water base value,CT value and energy spectrum curve slope at 40 keV(P>0.05).(2)The CTDIvol,DLP and ED values of low-dose spectral CT were lower than those of conventional spectral CT,and the differences were statistically significant(P<0.01).(3)The subjective scores of pulmonary nodules in the two groups were all above 3 points,and the difference was not statistically significant(P>0.05).Conclusion Revolution CT spectral scanning combined with ASIR-V preiteration algorithm can significantly reduce the radiation dose compared with conventional spectral scanning without affecting the display of pulmonary nodules and the measurement of spectral parameters,which has obvious advantages.
