干扰素-γ、白细胞介素-21水平与男性特发性不育症患者精液质量的相关性

摘　　要：目的观察干扰素-γ(IFN-γ)、白细胞介素-21(IL-21)水平与男性特发性不育症患者精液质量的关系。方法采用前瞻性队列研究,将南阳市第一人民医院生殖医学科2021年1月至2022年6月接诊的男性特发性不育症患者作为本次研究对象,于患者就诊时自制一般资料调查问卷,调查患者人口学一般资料;采用酶联免疫吸附法测定血清IFN-γ、IL-21水平;同时进行精液常规和精浆生化检查,并根据检查结果分为少精子症组、弱精子症组、畸形精子症组。比较3组人口学资料、精浆生化指标[果糖、中性α-葡糖苷酶(NAG)]及IFN-γ、IL-21水平;采用双变量Pearson直线相关性检验分析IFN-γ、IL-21水平与男性特发性不育症患者精液质量的关系。结果共纳入107例男性特发性不育症患者,研究期间2例主动退出研究,最终105例患者纳入本次研究。105例患者中,39例少精子症,44例弱精子症,22例畸形精子症;3组精子浓度、前向运动精子比率、精子活动率、精子畸形率、精浆果糖、精浆NAG、精浆IFN-γ、精浆IL-21比较,差异有统计学意义(P<0.05);3组其他资料比较,差异无统计学意义(P>0.05);双变量Pearson直线相关性检验显示,精浆IFN-γ与精子浓度、精子活动率呈负相关(r<0,P<0.05);精浆IL-21与前向运动精子比率、精子活动率呈负相关(r<0,P<0.05);精浆IFN-γ、IL-21与精浆NAG均呈正相关(r>0,P<0.05)。结论精浆IFN-γ、IL-21与男性特发性不育症患者的精液质量密切相关。Objective To observe the relationship between interferon-γ(IFN-γ),interleukin-21(IL-21)levels and semen quality in male patients with idiopathic infertility.Methods A prospective cohort study was conducted to investigate the general demographic data of male patients with idiopathic infertility who were admitted to the Department of Reproductive Medicine of Nanyang First People’s Hospital from January 2021 to June 2022.The levels of serum IFN-γand IL-21 were measured by enzyme-linked immunosorbent assay.At the same time,semen routine and seminal plasma biochemical examination were carried out,and according to the examination results,they were divided into oligospermia group,asthenospermia group and missperm group.The demographic data,seminal plasma biochemical indexes[fructose,neutral alpha glucosidase(NAG)]and IFN-γ,IL-21 levels were compared among the three groups.Bivariate Pearson linear correlation test was used to examine the relationship between IFN-γ,IL-21 levels and semen quality in male patients with idiopathic infertility.Results A total of 107 male patients with idiopathic infertility were included.During the study period,2 patients voluntarily withdrew from the study,and 105 patients were finally included in the study.Among 105 patients,39 were oligospermia,44 were asthenospermia,and 22 were dysspermia.There were significant differences in sperm concentration,forward motile sperm ratio,sperm motility rate,sperm deformity rate,seminal plasma fructose,seminal plasma NAG,seminal plasma IFN-γ,seminal plasma IL-21 among the three groups(P<0.05).There was no significant difference in other data among the three groups(P>0.05).Bivariate Pearson linear correlation test showed that seminal plasma IFN-γwas negatively correlated with sperm concentration and sperm(r<0,P<0.05).Seminal plasma IL-21 was negatively correlated with the proportion of forward motile sperm and sperm motility(r<0,P<0.05).Seminal plasma IFN-γ,IL-21 were positively correlated with seminal plasma NAG(r>0,P<0.05).Conclusion Seminal pl
