基于“动-定序贯八法”辨治急慢性鼻炎

摘　　要：“动-定序贯八法”是范冠杰教授总结前贤治疗消渴病的学术思想所创立的思维方法,该学术思想的主要内容是辨识核心症状和核心病机,认为证候不断演变,兼夹症也不断变化,但是核心病机是相对固定的,要变中有守、定中有动地动态把握疾病核心病机的演变规律,从而有规律地施以相应的药串。该思维方法在提出之始主要指导消渴病的诊治,后在指导中医临床实践中不断完善,丰富了对银屑病、抑郁症等其他疾病的认识与治疗,形成了完整的学术体系。笔者在临床跟诊中发现范冠杰教授纯中医辨治急慢性鼻炎临床有起效快、不易复发的优点。文章尝试总结范教授辨治鼻炎的经验,从津液代谢的角度探讨中医认识急慢性鼻炎的病机:疾病是“动”的,不同阶段有不同的证型、不同的症状;但疾病的发展过程也是“定”的,贯穿其中的核心病机津液的代谢问题是相对固定的,是有规律可循的。最后,通过临床病案分析进一步阐述“动-定序贯八法”的内核及临床分析过程。文章也尝试提醒我们年轻中医师,临床诊治不能只套用成方,更重要的是锻炼分辨核心症状、提炼核心病机的思维能力。The“dynamics-immobility-order-succession eight therapies”is a thinking method established by Professor FAN Guanjie based on summarizing the previous academic thoughts on treating diabetes.The main content of this academic thought is to identify the core symptoms and the core pathogenesis.It is believed that the symptoms are constantly evolving,and the syndrome is also constantly changing,but the core pathogenesis is relatively fixed.The evolution law of the core pathogenesis of the disease should be kept in the changing and dynamic.According to the core pathogenesis which is fixed and evolving,the corresponding medicine prescription can be applied regularly.This thinking method is initially proposed to guide the diagnosis and treatment of diabetes,and then continuously improves in the clinical practice of TCM,enriches the understanding and treatment of psoriasis,depression and other diseases,and forms a complete academic system.In the clinical follow-up,the author finds that Professor FAN Guanjie’s pure traditional Chinese medicine differentiation and treatment of acute and chronic rhinitis has the advantages of quick onset and rare recurrence.This paper tries to summarize Professor FAN’s experience in differentiating and treating rhinitis,and discusses the pathogenesis of acute and chronic rhinitis in traditional Chinese medicine from the perspective of body fluid metabolism:the disease is“dynamic”:it has different syndrome types and symptoms in different stages.However,the development of the disease is also“fixed”:the core pathogenesis is that the metabolism of body fluid is relatively fixed and rule-based.Finally,the kernel and clinical analysis process of the“dynamics-immobility-order-succession eight therapies”are further elaborated through a clinical record analysis.This article also tries to remind young TCM physicians that in clinical diagnosis and treatment,we should not only apply prescription,but more importantly,exercise the thinking ability to distinguish the core symptoms and ref
