摘　　要：目的:对比棍点理筋正骨手法联合腰痹通胶囊与单纯服用腰痹通胶囊治疗血瘀气滞型腰椎间盘突出症(Lumbar Disc Herniation,LDH)患者的临床疗效,为后续临床治疗提供参考。方法:选取2020年1月-2021年3月在广州中医药大学第一附属医院就诊的血瘀气滞型LDH患者85例,将患者分为甲组(46例)和乙组(39例)。甲组采取棍点理筋正骨手法联合腰痹通胶囊治疗;乙组单纯服用腰痹通胶囊治疗。收集两组性别、年龄、病程等基础资料,对比评估治愈率、总体有效率、视觉模拟评分量表(Visual Analogue Scale,VAS)分数、日本骨科协会评估治疗分数(Japanese Orthopaedic Association Scores,JOA)。结果:甲组比乙组治愈率及总体有效率更高(P<0.05)。治疗后,两组VAS评分均较治疗前降低(P<0.05),甲组VAS评分较乙组降低更显著,差异有统计学意义(P<0.05)。治疗后两组组内JOA评分较治疗前显著提高,差异均有统计学意义(P<0.05)。两组组间治疗前后JOA评分大致相同,差异均无统计学意义(P>0.05)。结论:棍点理筋正骨手法联合腰痹通胶囊和单纯服用腰痹通胶囊均可有效治疗血瘀气滞型LDH患者,但前者在缓解疼痛、改善腰椎功能、提高生活工作质量方面更具有优势。Objective:To compare the clinical effects of the Gundian Lijin Zhenggu manipulation(棍点理筋正骨手法)(stickpointing,tendons and bone-setting techniques)plus the Yaobitong capsules(腰痹通胶囊)and the Yaobitong capsules on blood stasis and Qi(气)stagnation type lumbar disc herniation.Methods:A total of 85 patients with blood stasis and Qi stagnation type lumbar disc herniation in the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine from January 2020 to March 2021 were selected and divided into group A(46 cases)and group B(39 cases).The treatment method in group A was the Gundian Lijin Zhenggu manipulation plus the Yaobitong capsules,and group B was simply taken the Yaobitong capsules.Baseline data such as gender,age,and disease course in the two groups was collected.Evaluation indicators such as cure rate,overall effective rate,Visual Analog Scale(VAS),and Japanese Orthopedic Association Assessment Treatment Score(JOA)were compared.Results:The cure rate and overall effective rate in group A were higher than group B(P<0.05).After treatment,the VAS scores in two groups were lower than before treatment(P<0.05),and the VAS score in group A was lower than group B more significantly(P<0.05).The JOA score after treatment in the two groups was significantly higher than before treatment(P<0.05).The JOA scores before and after treatment were roughly the same between the two groups(P>0.05).Conclusion:The Gundian Lijin Zhenggu manipulation plus the Yaobitong capsules and the Yaobitong capsules can be used to treat patients with blood stasis and Qi stagnation type lumbar disc herniation,but the former has more advantages in relieving pain,improving lumbar function,and improving the quality of life and work.
