摘 要:目的探讨低场强磁共振成像(MRI)对坐骨股骨撞击综合征(IFIS)的诊断价值。方法回顾性分析坐骨股撞击综合征患者32例54个髋关节(病例组)和正常髋关节50个(对照组)的MRI表现,利用独立样本t检验分析2组IFS及QFS的差异。采用受试者工作特征(ROC)曲线绘制股方肌间隙(QFS)、坐骨股骨间隙(IFS)诊断IFI的诊断价值。结果患侧IFS及QFS均出现狭窄,测量病例组IFS及QFS分别为(1.23±0.26)cm、(0.63±0.22)cm,对照组分别为(2.39±0.44)cm、(1.38±0.33)cm,2组之间比较差异具有统计学意义(P=0.001、P=0.010)。病例组IFS与QFS之间呈正相关关系(r=0.66,P<0.05)。QFS及IFS诊断IFIS患者的ROC曲线下面积(AUC)为0.846、0.833,QFS灵敏度及特异度为75.9%和88.6%,IFS灵敏度及特异度为84.2%和89.5%。结论在IFIS诊断中采用低场强MRI检出率较高,能显示IFS、QFS狭窄及股方肌异常,其中IFS和QFS具有较高诊断价值,是诊断IFIS的最佳检查方法。Objective The paper discussed diagnostic value of low field MRI in ischiofemoral impingement syndrome(IFIS). Methods MRI performance of 32 patients with sciatic femoral impact syndrome were reviewed,with 54 hips in case group and 50 normal hips in control group, the difference of IFS and QFS in both groups was analyzed by independent sample ttest. The ROC curve was used to draw diagnostic value of QFS and IFS in diagnosing IFI. Results The ipsilateral ischiofemoral space(IFS) and quadratus femoris space(QFS) were narrowed,the IFS and QFS in the case group were(1.23±0.26) cm,(0.63±0.22) cm, and those in the normal group were(2.39±0.44) cm and(1.38 ±0.33) cm, respectively, the difference between the two groups was statistically significant( P =0.001, P=0.010). There was a positive correlation between IFS and QFS in the case group(r=0.66, P<0.05). The area under the ROC curve(AUC) of IFIS patients diagnosed by QFS and IFS was 0.846 and 0.833, the sensitivity and specificity of QFS were 75.9% and 88.6%, and the sensitivity and specificity of IFS were 84.2% and 89.5%. Conclusion Low field MRI has a high detection rate in detecting of IFIS, it can show IFS, QFS stenosis and quadratus femoris abnormalities, it is of high diagnostic value, which are the best choices for the diagnosis of IFIS.
