摘 要:目的:探讨哮喘易感基因多态性与支气管哮喘患儿血清免疫球蛋白E(IgE)及嗜酸性粒细胞水平的关系。方法:选取在河南省儿童医院哮喘门诊就诊的哮喘患儿270例,其中男142例,女128例;年龄(35.83±26.52)个月。检测患儿外周血IgE水平及嗜酸性粒细胞水平,并采用口腔拭子核酸提取方法检测哮喘易感基因多态性。结果:哮喘患儿的白细胞介素-13(IL-13)基因的rs20541位点、IL-4基因的rs2243250位点、β2肾上腺素受体(ADRB2)基因的rs1042713位点、IgE高亲和力受体1β(FCER1B)基因的rs569108位点,细胞色素P450家族3亚家族A肽5(CYP3A5)基因的rs776746位点、花生四烯酸5脂氧合酶(ALOX5)基因的rs2115819位点、糖皮质激素诱导转录因子1(GLCCI1)基因的rs37973位点、IgE低亲和力受体II(FCER2)基因的rs28364072位点、白三烯C4合成酶(LTC4S)基因的rs730012位点、腺苷酸环化酶9(ADCY9)基因的rs2230739位点、促肾上腺皮质素释放激素受体2(CRHR2)基因的rs7793837位点、T-BOX转录因子21(TBX21)基因的rs2240017位点均存在3种基因型,其中FCER2基因rs28364072位点的3种基因型患儿间IgE水平差异具有统计学意义(P<0.001),其余位点不同基因型患儿间IgE水平差异没有统计学意义(P>0.05)。检测到的各位点基因型间嗜酸性粒细胞水平差异没有统计学意义(P>0.05)。结论:在支气管哮喘患儿中,FCER2基因rs28364072位点的GG基因型患儿IgE水平高于其他2种基因型;FCER2基因多态性筛选可能有助于预测基因患儿对抗IgE疗法的治疗反应。Objective:To investigate the correlations of asthma susceptibility gene polymorphism and serum immunoglobulin E(IgE),eosinophilic granulocyte levels in children with bronchial asthma.Methods:A total of 270children with asthma were selected in the asthma clinic of Henan Children’s Hospital,including 142 males and 128females,aged(35.83±26.52)months.Peripheral blood IgE levels and eosinophil levels was detected,and asthma susceptibility gene polymorphism was detected by oral swab nucleic acid extraction.Results:In children with asthma,the rs20541 site of interleukin-13(IL-13)gene,rs2243250 site of IL-4 gene,rs1042713 site ofβ2 adrenergic receptor(ADRB2)gene,rs569108 site of immunoglobin E(IgE)high-affinity receptor 1β(FCER1B)gene,rs776746 of cytochrome P450 family 3 subfamily A peptide 5(CYP3A5)gene,rs2115819 of arachidonic acid 5 lipoxygenase(ALOX5)gene,rs37973 of glucocorticoid-induced transcription factor 1(GLCCI1)gene,IgE low affinability receptor II(FCER2)gene rs28364072,leukotriene C4 synthetase(LTC4S)gene rs730012,adenylate cyclase 9(ADCY9)gene rs2230739,corticotropin-releasing hormone receptor 2(CRHR2)gene rs7793837,T-BOX transcription factor 21(TBX21)gene rs2240017.There were three genotypes in all of them.Among them,the children patients with IgE levels of FCER2 gene rs28364072 at 3 genotypes were statistically significant(P<0.001),while IgE levels of other genotypes were not statistically significant(P>0.05).There was no significant difference in IgE level among child patients with different genotypes at other sites(P>0.05).Conclusions:In children with bronchial asthma,IgE level of FCER2 gene rs28364072GG genotype was higher than that of other two genotypes.FCER2 gene polymorphism screening may help predict patients’response to IgE therapy.The FCER2 gene polymorphism screen may help predict the child patients’response to anti-IgE therapy.
