人工真皮替代物治疗足趾并趾畸形中皮肤缺损的临床应用研究

摘　　要：目的探究采用人工真皮替代物皮耐克TM(以下简称:皮耐克TM)在足趾并趾畸形手术中治疗皮肤缺损的应用效果。方法针对足趾并趾畸形手术中存在的皮肤缺损问题,本团队采用一期皮耐克TM替代传统自体皮移植的方法。2016年9月至2019年9月,于华中科技大学同济医学院附属协和医院手外科对24例患者(38个趾蹼)应用皮耐克TM修复分趾后的皮肤缺损。评估术后温哥华疤痕量表(Vancouver Scar Scale,VSS)、趾蹼爬移等级、视觉模拟评分法(visual analogue scale,VAS)评分。结果术后平均随访21.75(12~35)个月。经统计:VSS评分平均为0.37(0~2)分,趾蹼爬移评分平均为0.11(0~1)分。患者及其监护人对外观的VAS评分平均为0.16(0~1)分。结论在足趾并趾畸形的手术中应用皮耐克TM的治疗方案,操作简单、易于实施,同时能够重建外形良好的趾蹼,从而避免了植皮,为足趾并趾畸形中的皮肤缺损治疗提供了一种可供选择的好方法。Objective To investigate the effect of the artificial dermis substitute PelnacTM(hereinafter referred to as PelnacTM)in the treatment of skin defects in the operation of toe syndactyly.Methods In order to solve the problem of skin defect in the operation of syndactyly,we used one-stage skin PelnacTMinstead of traditional autologous skin graft.From September 2016 to September 2019,we applied PelnacTMto 24 patients(38 toe webs)in the Hand Surgery Department of Union Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology to repair the skin defect after toe separation.The postoperative Vancouver Scar Scale(VSS),toe web crawling grade and visual analog scale(VAS)were evaluated.Results The average follow-up was 21.75(12~35)months.According to statistics,the average VSS score was 0.37(0~2)points,and the average toe web crawling score was 0.11(0~1)points.The average VAS score of patients and their guardians was 0.16(0~1)points.Conclusion The application of the PelnacTMin the operation of syndactyly is simple and easy to implement.At the same time,it can reconstruct the toe web with good shape,thus avoiding skin grafting,and providing a good alternative method for the treatment of skin defects in the syndactyly.
