摘  要:目的比较单纯哮喘患者和哮喘合并支气管扩张患者气道微生物组的特征。方法根据标准入组30例单纯哮喘和25例哮喘合并支气管扩张患者, 分别进行肺功能检测和痰诱导试验, 挑取有效痰组织, 进行细菌16SrRNA测序, 下机数据与GreenGenes物种分类数据库进行比对和物种注释, 比较单纯哮喘组与哮喘合并支气管扩张组诱导痰样本细菌的多样性、丰度和菌群特征。结果与单纯哮喘患者相比, 哮喘合并支气管扩张患者其病程较长、BMI较低、肺功能和日常症状控制更差(P<0.05或P<0.01)。哮喘合并支气管扩张患者诱导痰中的细菌多样性呈现下降趋势(P=0.192或P=0.203), 变形菌门的相对丰度升高(P<0.05), 而肠杆菌科、莫拉菌科和假单胞菌科相对丰度显著升高(P<0.05或P<0.01)。结论与单纯支气管哮喘患者相比, 哮喘合并支气管扩张患者其诱导痰中以肠杆菌科、莫拉菌科和假单胞菌科为主的潜在致病菌增多。Objective We sought to characterize and compare the microbiome of induced sputum in asthmatic and asthma combine with bronchiectasis.Methods A total of 30 patients with simple asthma and 25 patients with asthma complicated with bronchiectasis were enrolled.Pulmonary function test and sputum induction test were performed respectively.Bacterial 16SrRNA sequencing was performed on valid sputum tissues,and the sequencing data were aligned with the GreenGenes taxonomic database and species annotated.The diversity,abundance and characteristics of the sputum microbiome were compared between the simple asthma group and the asthma combined with bronchiectasis group.Results Patients with asthma and bronchiectasis had longer disease duration,lower BMI,and poorer lung function and daily symptom control than patients with asthma alone(P<0.05 or P<0.01).Compared with patients with simple asthma,the airway bacterial diversity in patients with asthma combine with bronchiectasis decreased(P=0.192 or P=0.203),the relative abundance of Proteobacteria increased(P<0.05),and the relative abundance of Moraxellaceae,Pseudomonas and Enterobacteriaceae were significantly increased(P<0.05 or P<0.01).Conclusion In conclusion,potential pathogens of Enterobacteriaceae,Moraxellae and Pseudomonas were significantly increased in the airways of asthma combine with bronchiectasis.
