摘 要:目的探讨人乳腺珠蛋白(hMAM)、CXC趋化因子配体16(CXCL16)及特异性周期蛋白D1(Cyclin D1)表达水平在乳腺癌组织表达的临床意义。方法选取40例恶性乳腺癌患者(试验组)、40例良性乳腺癌患者(对照组)和40例健康体检者(正常组)为研究对象,检测3组受试者外周血清中hMAM mRNA、CXCL16和Cyclin D1水平。分析3种标志物与恶性乳腺癌患者TNM分期、淋巴结转移的相关性,并分析血清hMAM mRNA、CXCL16和Cyclin D1水平用于乳腺癌良恶性诊断的有效性。结果试验组、对照组和正常组的血清hMAM mRNA相对表达水平分别为6.54±2.50,4.27±1.14和1.00±0.05,CXCL16分别为(71.14±20.55),(45.71±12.22)和(13.99±4.00)pmol·L^(-1),Cyclin D1分别为(60.91±16.37),(43.86±12.12)和(14.61±2.98)U·L^(-1),两两比较,差异均有统计学意义(均P<0.05)。随着乳腺癌患者的TNM分期越高,血清hMAM mRNA、CXCL16和Cyclin D1的表达水平亦逐渐升高(均P<0.05);发生淋巴结转移的乳腺癌患者的血清hMAM mRNA、CXCL16和Cyclin D1的表达水平均较未发生淋巴结转移的高(均P<0.05)。针对患者肿瘤良恶性诊断,血清hMAM mRNA、CXCL16和Cyclin D1的受试者工作曲线下面积分别为0.76,0.84和0.80,最佳临界值分别为6.03,58.66 pmol·L^(-1)和53.21 U·L^(-1)。结论血清hMAM mRNA、CXCL16和Cyclin D1水平在与乳腺癌的早期诊断中具有重要意义,可以有效辅助确定乳腺癌的良恶性。Objective To investigate the expression levels of human mammaglobin(h MAM),C-X-C motif chemokine ligand 16(CXCL16)and Cyclin D1 in breast cancer tissue and their clinical significance.Methods A total of 40 patients with malignant breast cancer(treatment group),40 patients with benign breast cancer(control group)and 40 healthy people participating in physical examination(normal group)were selected as the research objects.The levels of h MAM mRNA,CXCL16 and Cyclin D1 in peripheral serum of three groups of subjects were detected.The correlation of the three markers with TNM stage and lymph node metastasis in patients with malignant breast cancer was analyzed,and the effectiveness of serum h MAM mRNA,CXCL16 and Cyclin D1 levels in the diagnosis of benign and malignant breast cancer was studied.Results The relative expression levels of serum h MAM mRNA in the treatment,control and normal groups were 6.54±2.50,4.27±1.14 and1.00±0.05,CXCL16 were(71.14±20.55),(45.71±12.22)and(13.99±4.00)pmol·L^(-1),the Cyclin D1were(60.91±16.37),(43.86±12.12)and(14.61±2.98)U·L^(-1),respectively.The differences were statistically significant in pairwise comparison among the three groups(all P<0.05).With the higher TNM stage of breast cancer patients,the expression levels of serum h MAM mRNA,CXCL16 and Cyclin D1 were also gradually increased(all P<0.05).The expression levels of serum h MAM mRNA,CXCL16 and Cyclin D1 were higher than those without lymph node metastasis(all P<0.05).For benign and malignant cancer diagnosis in patients,the areas under the receiver operating curve of serum h MAM mRNA,CXCL16 and Cyclin D1 were 0.76,0.84 and 0.80,the best cut-off values were 6.03,58.66 pmol·L^(-1),and 53.21 U·L^(-1),respectively.Conclusion The levels of serum h MAM mRNA,CXCL16 and Cyclin D1 are important in the early diagnosis of breast cancer and can effectively assist in determining benign and malignant cancer.
