全频下降型突发性聋预后的相关因素分析

摘　　要：目的分析全频下降型突发性聋患者预后相关的影响因素。方法选择收治的78例全频下降型突发性聋患者,分为治疗有效和无效两组。比较血液学检验相关参数的差异。分别采用Logistic回归和ROC曲线分析上述参数对于全频下降型突发性聋预后的影响及预测价值。结果全频下降型突发性聋治疗有效组纤维蛋白原和C-反应蛋白水平明显低于治疗无效组(P分别为0.001和0.025)。Logistic回归分析发现,只有纤维蛋白原水平能显著影响其预后(P=0.032)。而且,纤维蛋白原水平对全频下降型突发性聋有显著的预测价值(P=0.001),预测的敏感性和特异性分别为85.4%和60.0%。结论血清纤维蛋白原水平与全频下降型突发性聋的预后有明显相关性,可作为评估其预后的临床应用指标。OBJECTIVE To analyze prognostic factors in patients with full-frequency sudden sensorineural hearing loss(SSNHL).METHODS 78 patients with fullfrequency SSNHL admitted to our department were selected.They were divided into two groups:the effective and ineffective group.Differences in blood test between effective and ineffective group were compared.Logistic regress analysis was used to analyze the influence of the above factors on the prognosis of full-frequency SSNHL.And the ROC curve was also performed to evaluate the prognostic value of the above factors on the prognosis of all-frequency SSNHL.RESULTS There were significant differences in fibrinogen and CRP level between the two groups(P=0.001 and 0.025,respectively).Logistic regression analysis showed that fibrinogen level significantly affected the prognosis of full-frequency SSNHL(P=0.032).Furthermore,fibrinogen level had a significant predictive value for the prognosis of full-frequency SSNHL(P=0.001).According to ROC curve,the sensitivity and specificity of serum fibrinogen level in predicting the prognosis of full-frequency SSNHL were 85.4%and 60.0%,respectively.CONCLUSION The serum fibrinogen level can be considered as a valuable predictor for the prognosis of full-frequency SSNHL.
