摘 要:目的探讨耳蜗大小及形态评估对微小耳蜗畸形诊断及人工耳蜗植入术的意义。方法42例(84耳)CT示耳蜗结构正常的先天性极重度感音神经性聋儿童为人工耳蜗植入术(cochlear implantation,CI)组,42名(84耳)听力正常儿童为对照组,在耳蜗标准坐标系统上测量耳蜗大小:耳蜗底转长径、底转宽径、耳蜗高度、不同位置(180°、270°、360°、720°)耳蜗长度及耳蜗全长;耳蜗形态:耳蜗底转倾斜角、一二转间夹角及底转形状。比较各参数在两组间的差异。结果CI组耳蜗大小参数均小于对照组(P<0.05)。耳蜗形态参数两组间无差异(P>0.05)。结论耳蜗大小评估对诊断微小耳蜗畸形及人工耳蜗电极个性化选择具有指导意义,耳蜗形态评估有助于预测人工耳蜗手术难易。OBJECTIVE To investigate the significance of cochlear size and morphology assessment for the diagnosis of tiny cochlear malformation and cochlear implantation.METHODS 42 cases(84 ears)of congenital severe sensorineural hearing loss children with grossly normal cochlea image were included in this study.42 cases(84 ears)of children with normal hearing were compared as control.The measurements were performed on consensus panels of the cochlear coordinate system.Parameters of cochlear size:length of cochlear base,width of cochlear base,cochlear height,cochlear length in different positions(180°,270°,360°,720°)and the whole cochlear length.Parameters of cochlear morphology:tilt angle within the first turn of cochlea,angle between the first and second turn of cochlea,shape of cochlear base.RESULTS The parameters of cochlear size in CI group were smaller than those in the control group(P<0.05),while the parameters of cochlear morphology have no statistically significant differences with those in control group(P>0.05).CONCLUSION Measuring the parameters of cochlear size on consensus panels of the cochlear coordinate system can help diagnose the tiny cochlear malformation.It is also necessary for personalized cochlear implant.Measurement of cochlear morphological parameters can help predict the ease of CI surgery.
