摘  要:目的 探析通过高频彩超多普勒超声与弹性成像技术联合在筛查微小乳腺癌与乳腺良性结节的临床价值。方法纳入在黄河三门峡医院2019年1月至2022年1月间接受诊疗的乳腺结节患者,乳腺良性结节与微小乳腺癌患者中分别随机抽取127例、115例。均给予高频彩色多普勒超声和弹性成像技术检查,探究联合检查方式对两类乳腺结节患者的筛查价值。结果 相对于单一高频彩色多普勒超声检查,联合弹性成像技术在病灶鉴别的敏感度与特异度明显更高,差异有统计学意义(P<0.05)。微小乳腺癌与乳腺良性结节在形态不规则、包膜不完整及边界毛刺检出率差异有统计学意义(P<0.05)。乳腺良性结节与微小乳腺癌在影像报告和数据系统评分差异有统计学意义(P<0.05)。微小乳腺癌患者检测收缩期血流峰速、阻力指数、弹性应变率所得数据均显著高于乳腺良性结节,差异有统计学意义(P<0.05)。结论 对乳腺结节患者在鉴别微小乳腺癌与良性结节时通过高频彩色多普勒超声与弹性成像技术联合有突出的敏感度与特异度。Objective To explore the clinical value of high-frequency color Doppler ultrasonography combined with elastic imaging in screening for small breast cancer and benign breast nodules. Methods Patients with breast nodules who received treatment in Sanmenxia Hospital of Yellow River from January 2019 to January 2022 were included.127 patients with benign breast nodules and 115 patients with minimal breast cancer were randomly selected. High-frequency color Doppler ultrasonography and elastic imaging were performed to explore the screening value of combined examination for two types of breast nodules. Results Compared with single high-frequency color Doppler ultrasonography, the sensitivity and specificity of combined elastic imaging were significantly higher in the identification of lesions, the difference was statistically significant(P<0.05). There were significant differences in the detection rates of irregular shape, incomplete envelope, and boundary burr between small breast cancer and benign breast nodules(P<0.05). There were statistically significant differences between benign breast nodules and minimal breast cancer in imaging reports and data system scores(P<0.05). The peak systolic blood flow velocity, resistance index, and elastic strain rate detected in patients with minimal breast cancer were significantly higher than those of benign breast nodules, with statistical significance(P<0.05). Conclusion The combination of high-frequency color Doppler ultrasound and elastic imaging has outstanding sensitivity and specificity in the identification of breast nodules from small breast cancer and benign nodules.
