曾朝辉治疗痹证验案二则

摘　　要：“痹证”一名最早见于《素问·痹论篇》,是在人体元气亏虚的基础上兼外感风寒湿热之邪引起的,相当于现代医学的风湿性疾病。该病以肌肉、关节、筋骨等处的疼痛、酸楚、重着、麻木、灼热、屈伸不利,甚至关节肿大变形等为主要的临床表现。痹证在《黄帝内经》中即以“风寒湿”作为痹证的基本病因,痹证也是中医药治疗的特色病种之一。导师曾朝辉是湖南中医药大学第三附属医院主任医师,教授,硕士研究生导师,全国中医特色技术传承骨干人才,株洲市名中医,中国中西结合学会疼痛专业委员会委员,湖南省中医药和中西医结合学会疼痛专业委员会常务委员,从医二十余载,融汇中西医学,贯通传统现代。他认为本病主要因正气亏虚,感受外邪,营卫不和所致,病位在肌肉、经络、关节,与肝、脾、肾关系密切,在治疗上根据痹证疼痛部位及程度轻重选药,将辨病与辨证相结合,在治疗上善用药对组方配伍,注意顾护脾胃,善用活血药、虫类药、藤类药、风药和对药,桂枝汤是《伤寒论》中调和营卫法的基础方,曾师擅用经典方,根据多年临床经验在临床中多选取桂枝汤加味来论治痹证,因人因证论治,临床随证加减,并强调应注意日常调护,效果显著。附验案2则以说明。“Bi syndrome”(痹证)first appears in Suwen(《素问》),which is caused by the deficiency of human vitality and exogenous wind-cold-damp-heat,which is equivalent to a rheumatic disease in modern medicine.The main clinical manifestations of the disease are pain,soreness,feeling heavy,numbness,burning,poor flexion and extension of muscles,joints,muscles and bones,and even swelling and deformation of joints.“Wind,cold and dampness”are the basic causes of arthralgia syndrome in Huangdi Neijing(《黄帝内经》),and arthralgia syndrome is also one of the characteristic diseases treated in traditional Chinese medicine.ZENG Zhaohui,the tutor,is the chief physician,professor,master’s supervisor of the Third Affiliated Hospital of Hunan University of Traditional Chinese Medicine,the backbone talent of the national characteristic technology inheritance of traditional Chinese medicine,a famous TCM doctor in Zhuzhou,a member of the Pain Professional Committee of the Chinese Association of Integrative Medicine and Western Medicine,and a standing member of the Pain Professional Committee of the Hunan Association of Traditional Chinese Medicine and Integrative Medicine.He has been practicing medicine for more than 20 years,integrating TCM and Western medicine,and connecting TCM and modern medicine.He believes that the disease is mainly caused by deficiency of vital energy,external evils,and disharmony between Yingqi and Weiqi(营卫不和).The disease is located in muscles,meridians,joints,and closely related to the liver,spleen,and kidney.In terms of treatment,drugs are selected according to the location and degree of arthralgia pain,and disease differentiation is combined with syndrome differentiation.In the treatment,drugs are well combined with prescriptions,and attention is paid to protect the spleen and stomach.Blood activating drugs,insect drugs,rattan drugs,wind drugs,and drug pairs are well used,the Guizhi decoction(桂枝汤)is the basic formula for harmonizing Ying(营)and Wei(卫)in Shanghan Lun(《伤
