舒劲主任医师治疗功能性便秘的临床经验

摘　　要：功能性便秘属于临床上常见的功能性胃肠疾病,是指排除全身器质性因素、结构异常、代谢障碍或其他原因所致的功能性肠病,临床表现主要为大便干结不通、排出困难、排便时间延长等特点。近几年来,随着人们饮食习惯的变化、生活节奏的加快以及社会心理因素的影响,功能性便秘的患病率在国内甚至是国际上都呈逐年递增趋势,有关国内外功能性便秘的患病率报道存在差异,主要表现为女性患病率高于男性,城市患病率高于农村,严重影响人们的生活质量,已经引起了国内外研究者的高度关注。西医治疗本病主要以泻药为主,疗效差且容易引起肠道菌群紊乱,中医治疗功能性便秘有明显的优势,能够控制本病的复发率,且疗效明显优于西医治疗。舒劲主任医师擅长中医药治疗消化系统疾病,在治疗功能性便秘的过程中十分强调因人制宜,病证结合,秉承“以补为通,通中寓补”的思想理念,在借鉴前人经验的基础上精准把握肾阳虚型功能性便秘的病机,结合自身临床实践自拟便秘方治疗肾阳虚型功能性便秘,疗效颇佳。文章通过对肾阳虚型功能性便秘中西医病因、病机方面的阐述,分别从大肠、肾与功能性便秘的关系进行研究,展开阐述舒劲主任医师治疗功能性便秘的研究经验,以期为功能性便秘的研究提供更多的思路与方向。Functional constipation is a common clinical functional gastrointestinal disease,which refers to the functional bowel disease caused by the exclusion of systemic organic factors,structural abnormalities,metabolic disorders or other reasons.The clinical manifestations are mainly dry stool obstruction,difficult excretion,prolonged defecation time and other characteristics.In recent years,with the influences of the change people’s eating habits,the acceleration pace of life and social and psychological factors,the prevalence of functional constipation has been increasing year by year in China and even in the world.There are differences in the prevalence of functional constipation at home and abroad.The prevalence of functional constipation in women is higher than that in men,and that in urban areas is higher than that in rural areas,which seriously affects people’s quality of life and has attracted great attention from researchers at home and abroad.Western medicine mainly uses laxative in the treatment of this disease,which has poor efficacy and is easy to cause intestinal flora disorder.Traditional Chinese medicine has obvious advantages in the treatment of the functional constipation,which can control the recurrence rate of this disease,and the curative effect is obviously better than that of Western medicine.Dr.SHU Jin is good at treating digestive diseases in traditional Chinese medicine.In the treatment of functional constipation,Dr.SHU emphasizes the individualized treatment of patients and the combination of disease and syndrome,adheres to the idea of“tonifying combined with dredging”,and on the basis of learning from previous experience,accurately grasps kidney Yang(阳)deficiency as the pathogenesis of functional constipation,and combined with its own clinical practice,the Bianmi prescription(便秘)is given.The prescription has a good effect on functional constipation of kidney Yang deficiency.In this paper,the etiology and pathogenesis of functional constipation of kidney Yang deficiency type are
