防己茯苓汤辅治对难治性心力衰竭患者外周血单个核细胞孤儿核受体及心室重构的影响

摘　　要：目的:评估自拟防己茯苓汤加味辅助常规抗心力衰竭药物治疗对难治性心力衰竭(Refractory Heart Failure,RHF)患者外周血单个核细胞(Peripheral Blood Mononuclear Cell,PBMCs)孤儿核受体及心室重构的影响。方法:选取2019年2月―2021年6月在临沂市中心医院确诊的74例RHF患者,运用随机数字表法将纳入患者分为观察组和对照组,每组37例。对照组采用常规抗心力衰竭药物治疗,观察组在对照组常规抗心力衰竭药物治疗基础上联合自拟防己茯苓汤加味辅助治疗,两组均持续治疗1个月。分别于治疗前后采用实时荧光定量聚合酶链式反应法检测两组患者的PBMCs孤儿核受体表达水平;采用超声诊断仪检测两组患者的心室重构相关指标[左室内径缩短率(Left Ventricular Fraction Shortening,LVFS)、左室后壁厚度(Left ventricular Posterior Wall Thickness,LVPWT)及左心室收缩末期内径(Left ventricular end-systolic Diameter,LVESD)]水平。结果:观察组和对照组患者孤儿核受体表达水平低于治疗前,心室重构相关指标LVPWT、LVESD低于治前,LVFS高于治疗前(P<0.05)。观察组孤儿核受体表达水平低于对照组(P<0.05)。观察组LVFS高于对照组,LVPWT、LVESD低于对照组(P<0.05)。结论:自拟防己茯苓汤加味辅助常规抗心力衰竭药物治疗RHF患者可降低PBMCs孤儿核受体表达水平,改善心室重构。Objective:To investigate the effect of the Fangji Fuling decoction(防己茯苓汤)on solitary nuclear receptor of peripheral blood mononuclear cells(PBMCs)and ventricular remodeling in patients with refractory heart failure(RHF).Methods:A total of 74 patients with RHF diagnosed in Linyi Central Hospital of Shandong Province from February 2019 to June 2021 were randomly divided into the observation group and the control group,with 37 cases in each group.The control group was treated with conventional anti heart failure drugs,and the observation group was treated with the Fangji Fuling decoction plus conventional anti heart failure drugs,both groups were treated for 1 month.Before and after treatment,real-time fluorescence quantitative PCR was used to detect the expression level of PBMCs orphan nucleus receptor in the two groups,and ultrasonic diagnostic instrument was used to detect the level of ventricular remodeling related indexes in the two groups.Results:The expression level of PBMCs orphan nucleus receptor,left ventricular posterior wall thickness(LVPWT)and left ventricular end systolic diameter(LVESD)in the observation group were lower than those in the control group,and the level of left ventricular diameter shortening rate(LVFS)was higher than the control group(P<0.05).Conclusion:The Fangji Fuling decoction can reduce the expression level of orphan nucleus receptor in PBMCs and ameliorate ventricular remodeling in RHF patients.
