辨证耳穴压豆联合肝病治疗仪辅助治疗肝郁脾虚型慢性乙型病毒性肝炎患者胁痛的临床疗效观察

摘　　要：目的:观察辨证耳穴压豆联合肝病治疗仪辅助治疗肝郁脾虚型慢性乙型病毒性肝炎(Chronic Hepatitis B,CHB)患者胁痛的临床疗效。方法:将80例诊断为肝郁脾虚型CHB并伴有胁痛的患者随机分为两组,对照组在研究期间予以抗病毒、护肝降酶、口服中药汤剂等常规内科治疗,同时辅以肝病治疗仪缓解患者胁痛,并予常规护理。观察组则在对照组治疗基础上予辨证耳穴压豆,疗程均为2周,观察两组患者治疗前后胁痛改善情况、匹兹堡睡眠质量指数量表(Pittsburgh Sleep Quality Index,PSQI)评分、焦虑自评量表(Self-rating Anxiety Scale,SAS)及抑郁自评量表(Self-rating Depression Scale,SDS)评分及生活质量[慢性肝病问卷(Chronic Liver Disease Questionnaire,CLDQ)评分]。结果:治疗前,两组胁痛积分、PSQI、SAS/SDS、CLDQ评分的差异无统计学意义(P>0.05);治疗后两组胁痛积分、PSQI、SAS/SDS评分均明显低于本组治疗前,观察组分值降低更为显著;两组CLDQ评分明显高于本组治疗前,而观察组分值增高更为显著,差异有统计学意义(P<0.05)。结论:辨证耳穴压豆联合肝病治疗仪辅助治疗可明显改善肝郁脾虚型CHB患者的胁痛症状、睡眠质量,同时可以缓解焦虑、抑郁情绪,提高患者的生活质量。Objective:To observe the clinical efficacy of syndrome differentiation and treatment of auricular acupoint pressing therapy plus liver disease therapeutic instrument in treating hypochondriac pain of patients with chronic type B hepatitis due to stagnation of liver-Qi(气)and spleen deficiency.Methods:Eighty patients with CHB diagnosed hypochondriac pain due to stagnation of liver-Qi and spleen deficiency were randomly divided into two groups.The control group received conventional medical treatment such as antiviral,liver protection and enzyme lowering,oral TCM decoction,supplemented with liver disease treatment instrument to relieve hypochondriac pain,and routine nursing was given.On the basis of the treatment in the control group,the observation group received syndrome differentiation and treatment of auricular acupoint pressing therapy.The treatment course was 2 weeks.The amelioration of hypochondriac pain,sleep quality(PSQI),anxiety,depression(SAS/SDS)negative emotion score and quality of life(CLDQ score)of patients in the two groups were observed before and after treatment.Results:Before the treatment,the difference was not statistically significant in the comparison of hypochondriac score,PSQI,CLDQ and SAS/SDS score between the two groups(P>0.05).After the treatment,the hypochondriac score,PSQI,CLDQ and SAS/SDS scores were obviously lower than those before treatment,and the scores in the observation group were reduced more significantly.The CLDQ scores in the two groups were significantly higher than those before treatment,while the scores in the observation group were increased more significantly,and the differences were statistically significant(P<0.05).Conclusion:Syndrome differentiation and treatment of auricular acupoint pressing therapy plus liver disease therapeutic instrument can obviously relieve the symptoms of hypochondriac pain and improve sleep quality of CHB patients with hypochondriac pain due to stagnation of liver-Qi and spleen deficiency,meanwhile it can relieve anxiety,depression and imp
