参松养心胶囊对STEMI合并OSA急诊PCI术患者睡眠质量的影响

摘　　要：目的:探究参松养心胶囊对ST段抬高心肌梗死(STEMI)合并阻塞性睡眠呼吸暂停(OSA)急诊PCI术患者睡眠质量的影响。方法:选取2019年5月至2021年5月上海市嘉定区中医医院收治的STEMI合并OSA患者84例作为研究对象,采用随机数字表法分为对照组与观察组,每组42例,2组患者均接受PCI术与常规冠心病二级预防治疗,观察组在此基础上加用参松养心胶囊,比较2组患者心脑血管不良事件发生情况、多导睡眠指标与匹兹堡睡眠质量指数问卷(PSQI)评分。结果:2组患者脑卒中、血运重建等不良心脑血管发生事件发生率比较差异无统计学意义(P>0.05),观察组患者非致死性心肌梗死或再发心绞痛后又一次入院事件发生率与不良心脑血管发生事件总发生率观察组低于对照组,差异有统计学意义(P<0.05);治疗后,2组患者睡眠潜伏期、快速眼动睡眠潜伏期组内与组间比较差异均无统计学意义(均P>0.05);睡眠总时间均升高,观察组高于对照组,PSQI评分均降低,观察组低于对照组,差异均有统计学意义(均P<0.05)。结论:参松养心胶囊降低STEMI合并OSA心脑血管不良事件发生情况,提高睡眠质量,临床具有应用价值。Objective:To explore the effect of ShensongYangxin Capsules on sleep quality of patients with ST-segment elevation myocardial infarction(STEMI)and obstructive sleep apnea(OSA)treated with emergency PCI.Methods:A total of 84 patients with STEMI and OSA admitted to Jiading Hospital of Traditional Chinese Medicine from May 2019 to May 2021 were divided into control group and treatment group by random number table method,with 42 patients in each group.Both groups were given PCI and conventional secondary intervention for coronary heart disease.On this basis,patients in treatment group were treated with ShensongYangxin Capsules.The incidence of adverse cardiovascular and cerebrovascular events,polysomnography indexes and Pittsburgh Sleep Quality Index(PSQI)scores were compared between the two groups.Results:There was no significant difference in the incidence of stroke,revascularization and other adverse events between the two groups(P>0.05).The adverse rate of another hospitalization event after non-fatal myocardial infarction or recurrent angina pectoris and the total incidence of adverse events in the treatment group were lower than the control group(P<0.05).After treatment,there was no significant difference in sleep latency and REM sleep latency between the two groups between the groups(P>0.05).The total sleep time increased,and the treatment group was higher than that of the control group.The PSQI score decreased,and the treatment group was lower than that of the control group(P<0.05).Conclusion:ShensongYangxin Capsules can reduce adverse cardiovascular and cerebrovascular events and improve sleep quality of patients with STEMI and OSA.
