摘  要:目的探究可溶性B7-H3(sB7-H3)调控辅助性T细胞1(Th1)细胞介导免疫应答对肺炎支原体肺炎(MPP)患儿病情进展的影响。方法选取2019年7月-2021年7月焦作市人民医院收治的MPP患儿96例为MPP组,并招募同期体检健康的儿童100名为对照组,MPP组患儿根据发病期分为缓解期59例和急性期37例,根据病情严重程度分为轻症75例和重症21例,酶联免疫法检测血清sB7-H3、Th1细胞因子[白细胞介素-2(IL-2)和干扰素γ(IFN-γ)]、Th2细胞因子[IL-4和IL-10]水平,流式细胞术检测外周血Toll样受体4(TLR-4)、核因子κB(NF-κB)和髓样分化因子88(MyD88)表达情况。结果MPP组患儿血清sB7-H3、IL-4、IL-10、外周血TLR-4、NF-κB和MyD88水平高于对照组,IL-2和IFN-γ水平低于对照组,差异有统计学意义(P<0.05);急性期患儿血清sB7-H3、IL-4、IL-10、外周血TLR-4、NF-κB和MyD88水平高于缓解期,IL-2和IFN-γ水平低于缓解期,差异有统计学意义(P<0.05);重症患儿血清sB7-H3、IL-4、IL-10、外周血TLR-4、NF-κB和MyD88水平高于轻症患儿,IL-2和IFN-γ水平低于轻症患儿,差异有统计学意义(P<0.05)。结论MPP患儿血清sB7-H3水平上升,可能通过TLR-4/NF-κB信号通路,促进炎性因子释放,抑制Th1细胞因子表达,参与病情进展。OBJECTIVE To explore the influence of T helper cell 1(Th1)-mediated immune response regulated by soluble B7-H3(sB7-H3)on disease progression in children with Mycoplasma pneumoniae pneumonia(MPP).METHODS A total of 96 children with MPP who were treated in Jiaozuo People’s Hospital from Jul 2019 to Jul 2021 were assigned as the MPP group,meanwhile,100 healthy children who received physical examination were chosen as the control group.The children of the MPP group were divided into the remission stage group with 59 cases and the acute stage group with 37 cases according to the stage of onset and were divided into the mild group with 75 cases and the severe group with 21 cases according to the severity of disease.The levels of serum Sb7-H3,Th1 cytokines[interleukin-2(IL-2)and interferon-γ(IFN-γ)]and Th2 cytokines[IL-4 and IL-10]were detected by enzyme-linked immunosorbent assay;the expression levels of peripheral blood Toll-like receptor 4(TLR4),nuclear factorκB(NF-κB)and myeloid differentiation factor 88(MyD88)were detected by flow cytometry.RESULTS The levels of serum sB7-H3,IL-4,IL-10,peripheral blood TLR-4,NF-κB and MyD88 of the MPP group were significantly higher than those of the control group,while the levels of IL-2 and IFN-γof the MPP group were significantly lower than those of the control group(P<0.05).The levels of serum sB7-H3,IL-4,IL-10,peripheral blood TLR-4,NF-κB and MyD88 of the acute stage group were significantly higher than those of the remission stage group,while the levels of IL-2 and IFN-γof the acute stage group were significantly lower than those of the remission stage group(P<0.05).The levels of serum sB7-H3,IL-4,IL-10,peripheral blood TLR-4,NF-κB and MyD88 of the severe group were significantly higher than those of the mild group,while the levels of IL-2 and IFN-γof the severe group were significantly lower than those of the mild group(P<0.05).CONCLUSION The children with MPP show rise of serum sB7-H3,which may promote the release of inflammatory factors,inhibit the expression o
