摘 要:目的探讨运动疗法联合增强型体外反搏(EECP)对冠状动脉粥样硬化性心脏病(CHD)患者心脏功能以及生活质量的影响。方法选取CHD心脏康复患者90例,按照治疗方法不同分为对照组、运动组、联合组各30例。对照组接受CHD常规治疗,运动组在常规治疗基础上给予运动康复,联合组在运动组的基础上给予EECP治疗,疗程12周。治疗前后观察三组6 min步行距离(6-MWD)、左室射血分数(LVEF)、汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分、生活质量调查表(SF-36)评分变化。结果90例患者因无法完成治疗丢失3例,最终纳入87例(对照组30例、运动组28例、联合组29例)。三组治疗前性别、年龄、BMI、CHD危险因素、用药情况等基线资料以及6-MWD、LVEF、HAMA评分、HAMD评分、SF-36评分比较差异均无统计学意义(P均>0.05)。治疗12周比较,联合组LVEF、HAMA评分、HAMD评分、SF-36评分优于运动组,联合组、运动组6-MWD优于对照组,差异均有统计学意义(P均<0.05)。结论运动疗法联合EECP可明显提升CHD患者的运动能力、心脏收缩功能,改善焦虑抑郁情绪和生活质量。Objective To investigate the effects of exercise therapy combined with enhanced external counterpulsation(EECP)on cardiac function and quality of life in patients with coronary heart disease(CHD).Methods Ninety patients with CHD were selected and divided into the control group,exercise group and combined group according to different treatment methods.Patients in the control group received routine treatment of CHD,patients in the exercise group received exercise rehabilitation on the basis of routine treatment,and patients in the combined group received EECP treatment on the basis of the exercise group for 12 weeks.Six-minute walk distance(6-MWD),left ventricular ejection fraction(LVEF),Hamilton Anxiety Scale(HAMA)score,Hamilton Depression Scale(HAMD)score,and quality of life questionnaire(SF-36)score were observed before and after treatment in the three groups.Results Among 90 patients,3 cases could not complete the treatment,and finally 87 cases were included(30 cases in the control group,28 cases in the exercise group,and 29 cases in the combined group).There were no significant differences in sex,age,BMI,CHD risk factors,medication status,6-MWD,LVEF,HAMA score,HAMD score,or SF-36 score among the three groups before treatment(all P>0.05).After 12 weeks of treatment,LVEF,HAMA score,HAMD score and SF-36 score of the combined group were better than those of the exercise group and the control group,with statistically significant difference(all P<0.05).Conclusion Exercise therapy combined with EECP can significantly improve the motor function,cardiac systolic function,anxiety,depression and quality of life of CHD patients.
