摘　　要：目的探讨人疱疹病毒6型(HHV 6)感染及白细胞介素-18(IL-18)基因多态性与类风湿性关节炎(RA)易感性的关联。方法选取2019年12月-2021年12月丽水市人民医院收治的80例RA患者为研究对象,为RA组,另选取同期80名健康体检者为对照组,采用巢式聚合酶链式反应(Nested-PCR)法检测外周血单个核细胞(PBMCs)、血浆中HHV 6 DNA,采用PCR定量扩增技术检测IL-18基因启动子区-607C/A位点、-137G/C位点基因多态性,采用多因素Logistic回归分析探究HHV 6感染、IL-18基因多态性与RA易感性的关联。结果两组PBMCs中HHV6 DNA阳性检出率比较,无统计学差异,但RA组血浆HHV6 DNA阳性检出率高于对照组(P<0.05);血清类风湿因子(RF)浓度≥60 IU/ml患者血浆HHV6 DNA阳性率较<60 IU/ml高(P<0.05),疾病高活动度患者血浆HHV6 DNA阳性率较病情缓解、中低活动度高(P<0.05);两组IL-18-607C/A基因型分布及等位基因频率比较,无统计学差异,但RA组IL-18-137G/C的GC基因型、C等位基因频率均高于对照组(P<0.05);多因素Logistic回归分析显示,血浆HHV6 DNA阳性者较阴性者RA发病风险升高倍3.089倍(95%CI:1.289~7.405),携带IL-18基因-137G/C位点(GC+CC)基因型者较GG型者RA发病风险增加2.524倍(95%CI:1.338~4.764)。结论HHV6活动性感染、IL-18-137G/C基因多态性可能与RA易感性有关。OBJECTIVE To explore the association of human herpesvirus 6(HHV 6)infection and interleukin-18(IL-18)gene polymorphism with susceptibility to rheumatoid arthritis(RA).METHODS A total of 80 RA patients who were treated in Lishui People′s Hospital from Dec 2019 to Dec 2021 were recruited as the study subjects and assigned as the RA group,meanwhile,80 healthy people who received physical examination were chosen as the control group.The positive rate of HHV 6 DNA in peripheral blood mononuclear cells(PBMCs)and plasma was detected by nested polymerase chain reaction(nested-PCR),the gene polymorphisms at-607C/A locus and-137G/C locus in promoter region of IL-8 gene were detected by PCR quantitative amplification technique.Multivariate logistic regression analysis was performed for the relationship between the HHV infection,IL-8 gene polymorphisms and the susceptibility to RA.RESULTS There was no significant difference in the positive rate of HHV 6 DNA in PBMCs between the two groups,while the positive rate of plasma HHV 6 DNA of the RA group was higher than that of the control group.The positive rate of plasma HHV6 DNA of the patients with serum rheumatoid factor(RF)concentration no less than 60 IU/ml was higher than that of the patients with the serum RF concentration less than 60 IU/ml(P<0.05);the positive rate of plasma HHV6 DNA of the patients with high activity of disease was higher than that of the patients with disease modifying,moderate to low activity of disease(P<0.05).There were no significant differences in the frequencies of genotypes and alleles at-607C/A locus of IL-18 gene between the two groups,while the frequencies of GC genotype and C allele at-137G/C locus of IL-18 gene were higher in the RA group than in the control group(P<0.05).Multivariate logistic regression analysis indicated that the plasma HHV6 DNA-positive patients were 3.089 times the risk at RA as the negative patients(95%CI:1.289-7.405),the patients carrying with GC+CC genotype at-137G/C locus of IL-8 gene were 2.524 times the risk at
