摘　　要：目的分析载脂蛋白E(ApoE)、5-羟色胺2A(5-HT2A)受体基因多态性与病毒性脑炎患儿精神障碍的关联。方法收集2019年1月-2021年12月重庆两江新区第一人民医院102例病毒性脑炎患儿临床资料,根据其临床症状是否伴精神障碍分为重症组39例与轻症组63例,并纳入36名同期健康体检儿童作为对照组。比较三组ApoE基因与5-HT2A受体基因各位点基因型及等位基因频率分布特点,并分析ApoE、5-HT2A受体基因多态性与病毒性脑炎精神障碍患儿预后转归的关系。结果重症组ApoE基因rs429358位点TC/CC基因型频率及等位基因C频率高于轻症组及对照组(P<0.05),轻症组则高于对照组(P<0.05),病毒性脑炎ApoE基因rs429358位点TC/CC基因型患儿为重症的相对风险度(OR)=2.813。重症组5-HT2A受体基因rs6313位点TT基因型频率及等位基因T频率高于轻症组及对照组(P<0.05),轻症组则高于对照组(P<0.05),病毒性脑炎5-HT2A受体基因rs6313位点TT基因型患儿为重症的OR=2.375。重症组ApoE rs429358位点TC/CC基因型和5-HT2A受体rs6313位点TT基因型患儿精神障碍缓解时间、重症监护室住院时间、总住院时间更长(P<0.05)。结论ApoE rs429358及5-HT2A受体rs6313位点多态性与病毒性脑炎患儿精神障碍及预后密切相关,可为临床诊疗提供新方向。OBJECTIVE To analyze the association of gene polymorphisms of apolipoprotein E(ApoE)and 5-hydroxytryptamine 2A(5-HT2A)receptor with mental disorders in children with viral meningitis.METHODS The clinical data were collected from 102 children with viral meningitis who were treated in Chongqing Liangjiang New Area First People′s Hospital from Jan 2019 to Dec 2021,the children were divided into the severe group with 39 cases and the mild group with 63 cases according to the clinical symptom of mental disorders,meanwhile,36 children who received physical examination were chosen as the control group.The frequencies of genotypes and alleles at loci of ApoE gene and 5-HT2A receptor gene were compared among the three groups,and the relationship between the gene polymorphisms of ApoE and 5-HT2A and the treatment outcomes of the viral meningitis children with mental disorders was observed.RESULTS The frequencies of TC/CC genotype and allele C at rs429358 locus of ApoE gene were higher in the severe group than in the mild group and the control group(P<0.05),which were higher in the mild group than in the control group(P<0.05).The odds ratio(OR)of severe viral encephalitis was 2.813 in the children with TC/CC genotype at rs429358 locus of ApoE gene.The frequencies of TT genotype and allele T at rs6313 locus of 5-HT2A receptor gene were higher in the severe group than in the mild group and the control group(P<0.05),which were higher in the mild group than in the control group(P<0.05).The OR of severe viral encephalitis was 2.375 in children with TT genotype at rs6313 locus of 5-HT2A receptor gene.In the severe group,the remission time of mental disorders,length of intensive care unit stay and total length of hospital stay of the children with TC/CC genotype at rs429358 locus of ApoE gene and TT genotype at rs6313 locus of 5-HT2A receptor gene were longer(P<0.05).CONCLUSION The polymorphisms at rs429358 locus of ApoE gene and rs6313 locus of 5-HT2A receptor gene are closely associated with the mental disorders and prognosis
