基于SPO的消毒供应中心手术器械清洗质量管理在预防医院感染中的效果

摘　　要：目的探讨基于结构-过程-结果(SPO)模型的消毒供应中心手术器械清洗质量管理在预防医院感染中的效果。方法选取2019年6月-2021年3月江苏省连云港市第一人民医院对患者进行治疗后回收的手术器械800件,将这些手术器械随机分成两组,对照组和研究组各400件,对照组进行基础的冲洗消毒,研究组在对照组基础上,再严格按照SPO的标准进行冲洗消毒。比较对照组和研究组手术器械清洗达标率、手术器械消毒满意数和手术器械再次使用时医院感染情况。结果两组手术器械一般资料对比,无统计学差异,具有可比性。两组手术器械清洗效果评价:研究组手术器械清洗达标率高于对照组,手术器械消毒满意数所占比例高于对照组,手术器械再次使用时医院感染情况低于对照组,差异均具有统计学意义(P<0.05)。结论基于SPO的消毒供应中心手术器械清洗质量管理可以大幅度提高手术器械的清洗达标率,减少医院感染,在医院中对患者的治疗有积极作用,保证患者在进行手术后不会发生其他可避免的伤害。OBJECTIVE To explore the effect of cleaning quality management of surgical instruments in central sterile supply department(CSSD)based on structure-process-outcome(SPO)model on prevention of nosocomial infection.METHODS A total of 800 pieces of surgical instruments that were recycled from the First People′s Hospital of Lianyungang City were chosen as the study objects and randomly divided into the control group and the study group,with 400 pieces in each group.The control group was treated with basic rinsing for disinfection,while the study group was treated with rinsing for disinfection based on the standards of SPO on basis of the control group.The qualified rate of cleaning,satisfaction with disinfection of surgical instruments and incidence of nosocomial infection were observed and compared between the control group and the study group.RESULTS There was no significant difference in the baseline data between the two groups,and the baseline data were comparable.As compared with the cleaning effect of surgical instruments,the qualified rate of cleaning of the study group was higher than that of the control group,the proportions of satisfaction with disinfection of surgical instruments was higher in the study group than in the control group,the incidence of nosocomial infection of the study group was lower than that of the control group when the surgical instruments were used again,and there were significant differences(P<0.05).CONCLUSION The cleaning quality management of surgical instruments in CSSD based on SPO may remarkably raise the qualified rate of cleaning of surgical instruments and reduce the incidence of nosocomial infection,it has positive effect on treatment of patients and prevent the patients from avoidable harms when undergoing surgery.
