摘　　要：目的分析胃饥饿素、可溶性P选择素(sP-selectin)、Toll样受体2(TLR2)在血液透析巨细胞病毒(CMV)感染中的表达及临床意义。方法选取2019年8月-2020年12月于医院收治的血液透析患者93例作为研究组,其中未合并CMV感染患者56例,合并CMV感染患者37例,另选取同期40名健康体检者作为对照组。酶联免疫吸附法检测两组研究对象Ghrelin、sP-selectin、TLR2表达,Pearson相关性对Ghrelin、sP-selectin、TLR2相关性进行分析,探究Ghrelin、sP-selectin、TLR2在血液透析CMV感染中的相关性。结果与对照组相比,研究组患者血清中Ghrelin表达较低,sP-selectin、TLR2表达较高(P<0.05)。与未合并CMV感染患者比较,合并CMV感染患者Ghrelin表达较低,sP-selectin、TLR2表达较高(P<0.05)。Ghrelin、sP-selectin、TLR2联合检测对血液透析CMV感染的预测价值优于单一检测(P<0.05)。Ghrelin与sP-selectin呈负相关(r=-0.408,P=0.014);Ghrelin与TLR2呈负相关(r=-0.755,P=0.001);sP-selectin与TLR2呈正相关(r=0.649,P=0.001)。结论Ghrelin在血液透析CMV感染患者血清中低表达,sP-selectin、TLR2在血液透析CMV感染患者血清中高表达,Ghrelin、sP-selectin、TLR2可作为血液透析CMV感染风险预测因子,可作为血液透析CMV感染的血清检测指标。OBJECTIVE To study the effects of Ghrelin,Soluble P selectin(SP-selectin)and Toll-like receptor 2(TLR2)on Cytomegalovirus(CMV)infection after hemodialysis,and to analyze its clinical significance.METHODS A total of 93 hemodialysis patients admitted to the hospital from Aug 2019 to Dec 2020 were recruited as the study group,including 56 patients without CMV infection and 37 patients with CMV infection.Another 40 healthy volunteers under physical examination during same period were in the control group.The expression of Ghrelin,sP-selectin and TLR2 was detected between the two groups by enzyme-linked immunosorbent method.Pearson correlation analysis was used to explore the correlation between Ghrelin,sP-selectin and TLR2 and CMV infection on hemodialysis.RESULTS Compared with the control group,the serum expression of Ghrelin in the study group was lower,while the serum expression of SP-selectin and TLR2 was higher(P<0.05).Compared with patients with no CMV infection,patients with CMV infection had lower Ghrelin expression and higher expression of sP-selectin and TLR2(P<0.05).The combined detection of Ghrelin,sP-selectin,and TLR2 had a better predictive value for hemodialysis CMV infection than single test(P<0.05).Ghrelin was negatively correlated with sP-selectin(r=-0.408,P=0.014);Ghrelin was negatively correlated with TLR2(r=-0.755,P=0.001),and sP-selectin was positively correlated with TLR2(r=0.649,P=0.001).CONCLUSION Ghrelin is low expressed in serum of hemodialysis patients with CMV infection while TLR2 are high expressed.Ghrelin,SP-selectin and TLR2 can be used as risk predictors for CMV infection in hemodialysis patients.It can be used as a serum detection index of CMV infection in hemodialysis.
