摘  要:目的探讨Toll样受体和相关炎症因子在沙眼衣原体(Ct)或人乳头瘤病毒(HPV)感染中的作用。方法选取2019年10月-2020年10月在邢台市人民医院妇科就诊的202例感染者,根据研究对象感染病原体类型分为HPV组、Ct组、HPV&Ct组,同时选取无感染症状的同期女性健康体检者50名为对照组;收集入组患者血液样本、宫颈分泌物样本,检测感染病原体类型、血常规、血清炎症因子、T淋巴细胞亚群比例,并进行结果分析。结果与对照组相比,HPV组、Ct组、HPV&Ct组的WBC、LYM、NE、BAS水平均有不同程度的升高,与HPV组相比,HPV&Ct组WBC、NE、EOS水平升高(P<0.05);HPV&Ct组PCT、IL-10、NF-κB、TLR4水平高于HPV组,而CRP、TNF-α、IL-6低于HPV组(P<0.05);与对照组相比,HPV组、Ct组、HPV&Ct组的CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平均有不同程度的降低,CD_(8)^(+)水平升高(P<0.05)。结论Ct感染后,显著促进HPV感染者的PCT、IL-10、NF-κB、TLR4水平表达,抑制CD_(3)^(+)、CD_(4)^(+)分化,推测Ct感染改变了细胞因子分泌,使HPV感染进展中TLR4负向调控CRP、TNF-α、IL-6的表达,并通过减少抗原递呈细胞,抑制细胞介导的免疫反应,阻止机体对HPV的清除。OBJECTIVE To investigate the role of Toll-like receptors and related inflammatory factors in Chlamydia trachomatis(Ct)and human papillomavirus(HPV)infection.METHODS Total of 202 infected patients treated in the Obstetrics and Gynecology Department of Xingtai People′s Hospital from Oct 2019 to Oct 2020 were enrolled,which were divided into the HPV group,Ct group,and HPV&Ct group according to the type of pathogens they were infected with.And 50 healthy females who had no symptoms of infection during the same period were recruited in the control group.Blood and cervical secretions samples from the patients of the groups were collected to detect the types of infectious pathogens,blood routines,serum inflammatory factors,and proportions of T lymphocytes subgroups.RESULTS Compared with the control group,the levels of WBC,LYM,NE and BAS in the HPV group,Ct group and HPV&Ct group increased by various extents.Compared with the HPV group,the WBC,NE,and EOS levels in the HPV&Ct group increased(P<0.05).The levels of PCT,IL-10,NF-κB,and TLR4 in the HPV&Ct group were higher than those in the HPV group,while CRP,TNF-α,and IL-6 levels were lower than those in the HPV group(P<0.05).Compared with the control group,the levels of CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+)in HPV group,Ct group and HPV&Ct group were decreased by various degrees,while the level of CD_(8)^(+)was increased.CONCLUSION Ct infection can significantly promote the expression of PCT,IL-10,NF-κB,and TLR4 in the HPV infected patients,and inhibit the differentiation of CD_(3)^(+)and CD_(4)^(+)cells.It is speculated that Ct infection changes the secretion of cytokines and promotes TLR4 to negatively regulate expression of CRP,TNF-α,and IL-6 by reducing antigen presenting cells,inhibiting cell-mediated immune response and preventing the elimination of HPV.
