摘  要:目的分析白细胞介素-2(IL-2)基因多态性及其表达水平与支原体感染性盆腔炎(PID)易感性的相关性。方法选取2017年1月-2020年12月医院收治的95例支原体感染性PID患者为研究组,并选取80名健康女性为对照组,分析PID患者支原体感染情况,采用聚合酶链反应-限制片段长度多态方法对IL-2基因-384,-475,-631位点进行多态性分析,采用酶联免疫吸附法检测血清IL-2水平,采用Logistic回归分析评价IL-2基因多态性位点与支原体感染性PID易感性的关系,采用工作特异性曲线(ROC)评价血清IL-2水平对支原体感染性PID的诊断价值。结果95例PID患者中,以解脲支原体感染为主,占84.21%。研究组IL-2基因-384 SNP TT基因型及T等位基因频率高于对照组,GG基因型及G等位基因频率低于对照组(P<0.05)。研究组血清IL-2水平低于对照组(P<0.05)。IL-2基因-384 SNP位点TT基因型(OR=1.429,95%CI:1.143~1.787)患支原体感染性PID的风险升高,GG基因型(OR=-0.347,95%CI:0.567~0.880)患支原体感染性PID的风险降低(P<0.05)。ROC曲线分析显示,血清IL-2水平诊断支原体感染性PID的曲线下面积(AUC)为0.852,敏感性为86.00%,特异性为76.00%。结论引起盆腔感染的主要为解脲支原体,且IL-2基因多态性及其表达水平与支原体感染性PID相关,IL-2基因-384SNP位点T等位基因可能会增加发生支原体感染性PID的危险性。OBJECTIVE To analyze the correlation of interleukin-2(IL-2)gene polymorphism and expression level with susceptibility to mycoplasma infective pelvic inflammatory disease(PID).METHODS A total of 95 patients with mycoplasma infective PID admitted to the hospital between Jan 2017 and Dec 2020 were enrolled as the observation group.Another 80 healthy women were recruited as the control group.The status of mycoplasma infection in the observation group were analyzed.The polymerase chain reaction-restriction fragment length polymorphism method was used to analyze IL-2-384,-475 and-631 gene polymorphisms.Serum IL-2 level was detected by enzyme-linked immunosorbent assay.Logistic regression analysis was performed to analyze the relationship between IL-2 gene polymorphisms and the susceptibility to mycoplasma infective PID.The receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of serum IL-2 level for mycoplasma infective PID.RESULTS Ninety-five patients with PID were mainly infected with Ureaplasma urealyticum,accounting for 84.21%.The frequencies of TT genotype and T allele on IL-2-384 SNP in the observation group were higher than those in the control group,while the frequencies of GG genotype and G allele were lower than those in the control group(P<0.05).Serum IL-2 level in the observation group was lower than that in the control group(P<0.05).TT genotype on IL-2-384 SNP(OR=1.429,95%CI:1.143-1.787)significantly increased the risk for mycoplasma infective PID,while GG genotype(OR=-0.347,95%CI:0.567-0.880)significantly lowered the risk of mycoplasma infective PID(P<0.05).ROC curve analysis showed that the AUC of serum IL-2 level in the diagnosis of mycoplasma infective PID was 0.852.Its sensitivity and specificity were 86.00%and 76.00%,respectively.CONCLUSION The main cause of pelvic infection is Ureaplasma urealyticum.IL-2 gene polymorphism and expression level are related to mycoplasma infective PID.T allele on IL-2-384SNP may increase the risk for mycoplasma infective PID.
