基于“五脏六腑皆令人咳”疾病观的肾咳证治探析

摘　　要：“五脏六腑皆令人咳”疾病观是针对咳嗽的总体阐论,体现了中医整体与局部之间相互影响和相互关联。以五脏咳之肾咳为例,在梳理归纳总结古今文献基础上,阐发其历史沿革中的3种不同内涵,同时从肾令肺咳之机制和针药辨治两方面对“肾咳”进行深度剖析,认为除了精气血津液等精微物质失调之外,经筋失养亦为肾咳重要病理基础;治疗上以针灸调整筋脉、调节经络之气,以药物促进精微物质生成和敷布,二者将局部与整体治疗相结合,能够起到更好的治疗作用,体现了《黄帝内经》“五脏六腑皆令人咳,非独肺也”疾病观念对疾病诊疗的重要指导作用。That"Dysfunction of the five Zang-organs and six Fu-organs can lead to coughing"is a disease concept of the general exposition of cough.It reflects the interaction and correlation between the whole and the part of body.This article mainly talks about kidney cough.On the basis of combing and summarizing a large number of ancient and modern literatures,this paper expounds three different connotations of kidney cough at different times,analyzes the"kidney cough"from pathological mechanism and methods of treatment.In addition to the imbalance of the essence,Qi,blood and body fluid,the loss of nourishment of the meridians and tendons is also an important pathological basis for renal cough.In terms of treatment,acupuncture and moxibustion are used to adjust the muscles and meridians,regulate the Qi of the meridians,and drugs are used to promote the generation and application of fine substances.The combination of local and overall treatment can play a better therapeutic role,reflecting the important guiding role of the disease concept of"all five Zang-organs and six Fu-organs are coughing,not only the lung"in the Huangdi Internal Classic for disease diagnosis and treatment.
