18F-FDG PET/CT Findings of Extraskeletal Ewing’s Sarcoma/Primitive Neuroectodermal Tumor

摘  要:目的探讨骨外尤文肉瘤/外周原始神经外胚层肿瘤(ES/pPNET)的临床、病理特点及18F-FDG PET/CT表现。资料与方法回顾性收集2011年1月—2021年3月郑州大学第一附属医院收治的13例ES/pPNET患者的临床、病理及影像资料,观察其18F-FDG PET/CT表现,并分析其最大标准化摄取值(SUVmax)与病理学指标Ki-67的相关性,运用Cox回归分析肿瘤大小、有无转移、SUVmax、Ki-67、治疗方式等是否为预后的影响因素。结果13例患者确诊时6例发生转移(均为区域淋巴结转移、其中1例伴有肺转移)。18F-FDG PET/CT显像中病灶均可见不同程度摄取显像剂增多,SUVmax为2.0~14.5。SUVmax与Ki-67无相关性(r=0.376,P=0.205)。肿瘤最大径、边界是否清晰、有无转移、原发SUVmax、治疗方式是否单一以及Ki-67均非预后的影响因素(P>0.05)。结论ES/pPNET的临床及影像表现缺乏特异性,18F-FDG PET/CT显像可较为敏感地探测局部和远处转移,对疾病早期诊断、分期和治疗方式的选择均有重要价值。病理及基因检测是确诊本病的“金标准”。Purpose To investigate the clinical,pathology features and18F-FDG PET/CT imaging characteristics in patients with extraskeletal Ewing’s sarcoma/peripheral primitive neuroectodermal tumor(ES/pPNET).Materials and Methods Clinical and imaging data of 13 patients with ES/p PNET admitted from January 2011 to March 2021 in the First Affiliated Hospital of Zhengzhou University were retrospectively collected.The18F-FDG PET/CT imaging findings of ES/pPNET were analyzed to determine the correlations between max standard uptake value(SUVmax)in PET/CT imaging and Ki-67.In addition,Cox regression was used to analyze whether tumor size,metastasis,SUVmax,Ki-67 and treatment were prognostic factors.Results Among of 13 patients with ES/pPNET,metastases occurred in 6 cases at diagnosis(6 cases with regional lymph node metastasis,including 1 case with lung metastasis).All the lesions were showed increased uptake of imaging agents with varying degrees via18F-FDG PET/CT imaging,and the SUVmax ranged from 2.0 to 14.5.There was no significant correlation between SUVmax and Ki-67(r=0.376,P=0.205).Maximum tumor diameter,clear boundary,presence of metastasis,primary SUVmax,single treatment and Ki-67 were all not the prognostic factors(P>0.05).Conclusion ES/p PNET was with lack specificity of clinical and imaging findings.18F-FDG PET/CT imaging can detect local and distant metastasis more sensitively,which is of great value for early diagnosis,staging and selection of treatment measures.Pathological and gene testing are the gold standard for diagnosis.
