温阳活血通脉方联合温通消肿外敷方治疗四肢闭合性骨折临床研究  

Clinical Study on Wenyang Huoxue Tongmai Prescription Combined with Wentong Xiaozhong External Application Prescription for Limb Closed Fracture

摘　　要：目的:观察温阳活血通脉方联合温通消肿外敷方治疗四肢闭合性骨折的临床疗效。方法:采用随机数字表法将80例四肢闭合性骨折患者分为对照组与治疗组各40例。对照组给予口服双氯芬酸钠双释放肠溶胶囊治疗,治疗组在对照组基础上给予温阳活血通脉方联合温通消肿外敷方治疗。比较2组治疗前后中医证候积分、炎症介质、血液流变学与生长因子指标,并评价2组临床疗效及安全性。结果:治疗组总有效率92.50%,高于对照组75.00%(P<0.05),临床疗效优于对照组(P<0.05)。治疗后,2组中医证候积分、血浆黏度、全血高切黏度值、全血低切黏度值较治疗前降低,红细胞沉降率(ESR)升高(P<0.05),且治疗组治疗后上述指标改善优于对照组(P<0.05)。治疗后,2组血清细胞间黏附因子-1(sICAM-1)、白细胞介素-6(IL-6)、缓激肽(BK)、前列腺素E2(PGE2)水平均较治疗前降低(P<0.05),血管内皮生长因子(VEGF)、血小板衍生生长因子(PDGF)、成纤维细胞生长因子-1 (FGF-1)、胰岛素样生长因子-1 (IGF-1)水平升高(P<0.05),且治疗组治疗后上述指标改善优于对照组(P<0.05)。治疗组骨折临床愈合时间明显低于对照组(P<0.05),2组均未发生严重不良反应。结论:温阳活血通脉方联合温通消肿外敷方治疗四肢闭合性骨折疗效确切,能有效改善血液流变学及生长因子指标,减轻患者炎性反应,加快骨折愈合,安全性较高。Objective:To observe the clinical effect of the therapy of Wenyang Huoxue Tongmai Prescription combined with Wentong Xiaozhong external application prescription on limb closed fracture.Methods:A total of 80 patients with limb closed fracture were divided into the control group and the treatment group according to the random number table method,with 40 cases in each group.The control group was treated with diclofenac sodium dual release enteric-coated capsules,and the treatment group was additionally treated with Wenyang Huoxue Tongmai Prescription combined with external application of Wentong Xiaozhong Prescription based on the treatment of the control group.Before and after treatment,the traditional Chinese medicine syndrome(TCM) scores,inflammatory mediators,hemorheology and growth factors indexes were compared between the two groups,and the clinical effects and safety in the two groups were evaluated.Results:The total effective rate was 92.50% in the treatment group,higher than that of 75.00% in the control group(P<0.05).The clinical effect in the treatment group was better than that in the control group(P<0.05).After treatment,the TCM syndrome scores,plasma viscosity and values of high-shear whole blood viscosity and low-shear whole blood viscosity in the two groups were decreased when compared with those before treatment,and the erythrocyte sedimentation rate(ESR) in the two groups were increased when compared with those before treatment(P<0.05),and the improvement in the above five indexes in the treatment group was better than that in the control group(P<0.05).After treatment,the levels of serum soluble intercellular adhesion molecule-1(sICAM-1),interleukin-6(IL-6),bradykinin(BK) and prostaglandin E2(PGE2) in the two groups were increased when compared with those before treatment(P<0.05),and the levels of vascular endothelial growth factor(VEGF),platelet-derived growth factor(PDGF),fibroblast growth factor-1(FGF-1) and insulin-like growth factor-1(IGF-1) in the two groups were increased,and the improvement
